Benefits of Using the DualWave™ Arthroscopy Pump With ApolloRF® Probes

Introducing an enhanced benefit of the Arthrex DualWave arthroscopy pump. When the Arthrex DualWave is used in inflow and outflow mode. The rinse function is available. This function can be beneficial when using radiofrequency ablation devices. The rinse function of the DualWave is a timed increase of outflow suction provided by the pump through an accessory cannula, creating greater flow and fluid exchange with the blue cannula suction line connected. Simply activate the rinse function while using the RF probe. This increase in fluid exchange, in combination with the suction connected to the radiofrequency probe, can help maintain desired joint temperatures while maintaining distension within the joint space.

Cam Resection Using a Curved Hip Burr, the ApolloRF® H50 Probe, and DualWave™ Outflow Tubing

My name is Doctor Travis Menge from Grand Rapids, Michigan, and today we're looking at a right hip for an arthroscopic camera section and femoral plasty. We're viewing through the mid anterior portal and we're working through our anterolateral portal. As you can see, we've got a large cam lesion looking from anterior to posterior. I typically start off using the H50 RF wand to help delineate where the extent of my cam is. Again, from a posterior to anterior direction, this wand allows me to retract the distal leaflet of the capsule and evaluate the extent of the Cam lesion without removing excessive tissue from the capsular tissue. To get the appropriate visualization, once I identify where the Cam lesion is, I typically will use the h50 RF to debride and remove some of the overlying cartilage or fibrous tissue that is often covering the Cam lesion. And again you can see this large Cam lesion. And this h50 RF wand does a great job in helping you outline the extent of the Cam deformity. Once we've identified and outlined our Cam lesion and the area of resection I'll use the curved shaver. The Torpedo to gently remove some of the edges of our capsule while maintaining the surrounding normal, healthy tissue for later capsular repair. What's great about this Torpedo is number one. The curved angle allows you to reach posteriorly as well, as it's a very precise and accurate resection. You can see we're cleaning up the edges of that capsule, but we're not removing or debriding an excessive amount of capsular tissue, which could compromise our capsular repair at the end of the case. Additionally, the tapered nose of this blade allows me to accurately resect the tissue that I would like to without causing excessive resection of the surrounding healthy tissue. We're now using a 5.0mm curved bur to address and resect this cam deformity. What's great about the curvature of this bur is that it really allows you to work posteriorly, and especially posterolateral, where sometimes it's difficult to access and you can create a nice resection. Again, working from a more posterior to anterior direction as you recreate the normal offset of the femoral head and neck junction. Another great tip with this bur is you is you can change how aggressive you'd like to resect by either using forward, which is more aggressive, or reverse on the foot pedal, which is going to be more of a polishing type resection. So to start off, I'll typically use a more aggressive resection with forward as we outline and resect this cam deformity. And the curved burr has a unique advantage of allowing you not only to access that posterior cam, but also the anterior cam much better than a straight burr. Another benefit with the curved burr is you spend less time switching portals as you're working on resecting a cam, especially large cam, where you often have to switch portals multiple times throughout the case. As you can see with the curved burr, as we're working on that margin to reshape that femoral head neck junction, you get excellent contouring with very precise removal of that cam deformity as we work more posterior and posterolateral. This burr allows you to access a lot of that cam that you'd have a hard time normally accessing with a straight burr. And often I'll switch between our h50 RF wand to work posteriorly and posterolateral as we debride the fibrous tissue covering the cam lesion, and as we continue to work more posteriorly, we want to internally rotate the foot and bring the leg into extension to help access that area. Now we're going to switch back to the curved bur. You can see how this curve allows you to work posteriorly and get the hard to reach areas of a large cam. Now the bone on this cadaver is very soft, but you can see the accessibility that you get with this curved bur compared to a straight bur. Additionally, the DualWave outflow is keeping our visualization clean throughout the resection and allowing an optimal resection with minimal debris blocking our view. As you can see, the curved bur is resecting the bone very nicely and allowing access as we move anterior in the hip. Here we are with completion of our camera section. Using the curved bur, you can see the access that the bur gave us to go all the way posterior as we work anteriorly as well. You can see that labrum is repaired very nicely, and we've got a great suction seal between the labrum and the femoral cartilage.

DualWave™ Arthroscopy Pump Fluid Management System

I love the DualWave pump. I've been using it for over five years now, and it's really become an integral part of my O.R. setup. The thing I like the most about the DualWave pump is its consistency. The setup is easy. I can personalize my own settings, and regardless of where I'm operating, whether it's in the hospital setting or a surgical center, I know I'm going to get a nice reproducible visual field. So what I found lacking in other fluid management systems was that oftentimes I would get wide fluctuations in the fluid inside the joint space, too little fluid, and I would get bleeding. Too much fluid. I'd get a lot of soft tissue swelling and extravasation. Personally, I like to use the outflow on all my cases. I think that really allows me to optimize all the features of the pump. I can keep my suction toggle wide open and that minimizes the bleeding as well as the fluid fluctuations that I often saw before. It allows me to use different features, like the lavage feature, where I get a just a short time cycle of increased pressure, and also the rinse feature, which is just an increase in flow and allows me to clear debris and maintain a nice clear field.

I think there are a lot of benefits that my patients see as a result of the DualWave pump. First and foremost is I get a nice, consistent visual field. I get very little pressure fluctuations throughout my case. So I'm able to run my pump at a nice low setting, which leads to less soft tissue extravasation. A lot of times I'm able to use little or no tourniquet at all. And I think that improves post-operative pain and overall function. I think the benefits outweigh the cost when it comes to outflow tubing. We've been able to use much less fluid during our cases when we're using outflow. We are also able to improve our overall efficiency, which cuts our time down. And I think that leads to overall improved savings for both the healthcare system as well as to society. I prefer a three portal setup in the knee. I like to use an inflow cannula in the suprapatellar pouch, and I attach my outflow cannula to the scope itself. I think one of the additional features with having the inflow outflow system on the DualWave pump is. It allows me to minimize the extra things that I used to do to try to minimize bleeding. I no longer use epinephrine in my bags. I use normal room temperature saline during my cases and no longer have to use refrigerated or extra cold saline, which can sometimes cramp your hands. The DualWave pump helps us with more complicated cases. We're asking our surgeons to do more and more with arthroscopic techniques. Things like superior capsule reconstructions, massive rotator cuff repairs, multi ligamentous knee reconstructions are long and complicated cases. For those cases, it's really important to have a good, clear visual field and minimize the soft tissue swelling in order to maximize our efficiency. So I think as we increase the complexity of our cases, having the tools like the DualWave pump that are going to allow us to see what we're doing and spend the time and focus our time and energy on the case at hand, is going to be very important.

The DualWave™ arthroscopy pump is an integrated inflow and outflow fluid management system that may also be used as a simple inflow-only arthroscopy pump. The DualWave arthroscopy pump is a safe, reliable, user‐friendly system that maintains constant, non‐pulsed control of intraarticular rinsing and distention pressure throughout all phases of an arthroscopic surgical procedure. It is intended to provide continuous pulse‐free flow that reacts immediately to changes in the intraarticular pressure so that joint distention can be sustained even under high shaver extraction volumes or secondary outflow.

DualWave™ Arthroscopy Pump Configuration Tutorial

When the DualWave is first placed in the operating room. Properly locating and set up of the DualWave is imperative. You must always remember that all pumps are affected by the relative height to the joint space. The DualWave is designed to be at the same height as the joint space for every foot below or above. You must subtract or add approximately 22mm of mercury to the displayed pressure. The DualWave can be placed on Arthrex standard fluid management cart. Mount the DualWave by simply removing the rubber feet and placing the DualWave on the top shelf. Tighten the Ford 10 to 32 by 7/8 screws to prevent the pump from falling off the cart. Universal Shaver Detect enables the DualWave to react quickly to the increased suction while shaving. Two four foot cords come with each DualWave one red synergy cord for shaver detection, communication between the DualWave and the SynergyResection console, and a universal shaver detection cord for connection between a legacy shaver systems or competitive shaver systems. An additional eight foot version is available as needed with legacy shaver consoles or competitive shaver consoles. Make sure both consoles are off. Remove the current power cord from the shaver console and replace it with the female end of the shaver detection cord. Locate the shaver detection receptacle on the rear of the pump. Once inserted, rotate the exterior part of the plug until it cannot be pulled out. The DualWave is ready to be connected to the main power. It is important to remember that for universal shaver detection to work properly, the shaver console must be fully powered on prior to powering on the DualWave. When installing a DualWave pump with 1.8 software or higher, and a SynergyResection console with 2.3 software or higher, always use the red SynergyResection cord. This removes the need for the universal shaver detection cord and improved method of communication between the consoles. To initially set up the DualWave, first decide whether the procedure will require inflow only or inflow outflow mode in both modes. A main pump tubing is needed. Begin by removing the tubing using standard sterile procedure. You will find an orange clip that holds the tubing pressure line connector and the bag spikes together. Open the left side door by lifting up on the lever. Open the clip, removing the tubing pressure line connector. Make sure that the sensor is open by simply pushing on the bottom of the pump's pressure sensor coupler. Connect the tubing pressure line connectors to the pump. The LED on the pressure coupler will turn solid green when the tubing is properly connected. Match the green color with the green mark. Wrap the tubing over the wheel until the clear collar sits in the right side. Always verify that both collars are properly seated. Close the inflow tubing door. And spike the saline bags. Remember to clamp the second line when only spiking one bag of saline. The inflow tubing must be thrown out when disengaged. As an alternative to the main pump tubing, Arthrex has developed an all day tube set called ReDeuce tube set. A two part system that comes with a pump side tubing and the patient side. Tubing. Each patient side tube set comes with a built in check valve that prevents contamination of the pump side tube set. The large coupler is easy to use and comes with a replacement cover in each patient side tube set to protect the pump side tubing. For operating rooms with distances greater than 13ft between the DualWave and operative site and extension tubing is available. The extension tube set provides eight extra feet of tubing for a total of 21ft. The extension tubing also comes with a check valve. To allow are 6010 to be converted into an all day tube set, similar to reduce tubing to optimize the control over the fluid management of the joint space. Attach the outflow tubing. Set the new outflow tubing comes color coded to simplify setup. Begin by removing the outflow tubing using sterile practice in the sterile field, take the cassette and the red waste tubing and throw them off the sterile field to the circulating nurse. For easy installation of the cassette onto the DualWave. Move the rollers so that two wheels are flush. Extend the boot of the outflow cassette over the two wheels at the same time with one hand. Hold the boot in place and with the second hand, pull down on all three tubes, pushing the cassette in place. Close the door, making sure the lever locks properly in place. You will hear the outflow side suction tubing pinch roller calibrate at the top of the main screen. Double arrows will also be seen if only one arrow is still seen. Tap the cassette into place in the sterile field. Connect the clear shaver tube to the shaver. Suction on the back of the handpiece. Cut for optimal fitting. If a third portal is used, connect the blue cannula tubing. The combination tube set comes with. Both are 6021 and are 6430 packaged together. Wide tubing and waste bags are also available as needed.
