Die vertiefende Trochleaplastik hat das Ziel, die Trochleamorphologie anzupassen und die Form eines Gleitlagers zu erhalten. Dieses soll die Patellaluxation unterbinden und dem Patienten ermöglichen, Alltagsaktivitäten wieder nachgehen zu können.
Eine vertiefende Trochleaplastik erfordert präzise Operationstechniken, die eine der Indikation entsprechende Lösung ermöglichen. Mit Hilfe des neuen Instrumentariums für Trochleaplastik von Arthrex lässt sich eine Knochen-Knorpel-Schuppe von 3 oder 5 mm Dicke ablösen. Die 2.9 mm Fräse ermöglicht die Präparation des Knochens und die Wiederherstellung der Form eines knöchernen Gleitlagers. Arthrex PushLock®- oder SwiveLock®-Anker und resorbierbares Fadenmaterial sorgen für eine stabile Refixierung der Knorpel-Knochen-Schuppe auf der anatomisch geformten Trochlea.

The 4.5 mm-diameter bicortical post has an extremely low-profile head to reduce soft-tissue irritation. A 2.5 mm drill for bicortical post is used to broach the cortical bone, while the depth device provides accurate sizing information. The optional low-profile, spiked washer may be used in conjunction with the bicortical post for soft-tissue tendon fixation of soft tissue tendons directly to bone.&nbsp;
The posts and washers are manufactured using titanium ASTM F-136 alloy. The smooth, concave-shaped washer securely traps attached sutures and bulky suture knots atraumatically under the low-profile washer to eliminate potential soft-tissue irritation. The spiked washer facilitates direct soft tissue or ligament fixation to bone.
Ligament staples with a low-profile bridge reduce the frequency of secondary removal due to patient discomfort caused by soft-tissue irritation. The cobalt chrome spiked fixation staple has sharp leg points for easier penetration into cortical bone without predrilling. The staple driver with attachable impactor/extractor allows complete impaction since the staple driver tip is flush with the staple bridge. The staple seating punch may be used for further impaction. All staples are 20 mm.
Finally, the two-piece TensionLoc™ implant system is designed to fix suture during ligament reconstruction procedures. Using this technique, tibial suture fixation is shown for a single- or double-bundle anterior cruciate ligament reconstruction (ACLR). A unique alternative to standard suture buttons, the TensionLoc implant allows suture-button maintenance for suture tension adjustments during knot tying. The low-profile PEEK implant sits flush and securely within the tunnel.

Das Interesse an ACL-Primärrefixationen als Behandlung von ACL-Rupturen des Typs I und II nach Sherman nimmt zu. Eine sorgfältige Analyse früherer Daten hat ergeben, dass bestimmte Untergruppen von Rissen des vorderen Kreuzbandes, insbesondere proximale Risse mit guter Gewebequalität, bei der ACL-Primärrefixation positive klinische Ergebnisse aufweisen.1
Studien über die Ergebnisse von Operationen zur Erhaltung des verbliebenen vorderen Kreuzbandes (VKB) haben ermutigende Ergebnisse mit einem potenziell höheren Heilungserfolg und besseren funktionellen Ergebnissen im Vergleich zu Operationen mit Resektion des verbliebenen VKB gezeigt.2 Die Erhaltung des ursprünglichen VKB mit der Primärrekonstruktionstechnik kann kurzfristig einen klinischen Erfolg erzielen und zu guten klinischen Ergebnissen bei der 3-Jahres-Nachbeobachtung in dieser sorgfältig ausgewählten Patientengruppe führen.3,4
Die arthroskopische Primärrefixation ist ein minimalinvasiver Ansatz, da keine Transplantate verwendet werden müssen und, falls erforderlich, die Revisionsoperationen eher analog zu den Primärrekonstruktionen erfolgen.4&nbsp;Ziel des Eingriffs ist es, das noch vorhandene natürliche Band zu erhalten. Auf diese Weise bleiben auch die natürlichen Nervenenden und die Blutversorgung erhalten.5
