OsteoPrecision™ Graft Delivery Device

So to prepare the bone mixture, I place the DBM and mix in the the cortical fibers. The cortical fibers have a higher density of BMP than the cancellous bone, and the DBM is usually a mixture of. Of bone and a carrier, so this increases the percentage of bone in this mixture by adding the cortical fibers. So once this is mixed, it is placed into the OsteoPrecision graft delivery device. Delivery cannula tip is then tightened onto it and it's ready for. This is our simulated femoral tunnel that we're going to fill with our DBM. And cortical fiber mixture cannula is introduced into the tunnel. So the large plunger is used first. You can see the DBM mixture advancing into the cannula. And we'll start seeing it in arthroscopically. And as the bone is advanced you just gently pull the cannula back out to allow it to fill the tunnel or the defect. The reverse phase medium prevents it from being washed away with a saline that's used in arthroscopy. So in this case we've completely filled the femoral defect. So looking at it from the medial portal that's awesome.

Hand and Wrist

Mark Campbell, MD, (Phoenix, AZ) demonstrates the subtalar arthrodesis procedure with Arthrex Biologics and fixation with 6.7 mm Low Profile cannulated lag screws. He utilizes StimuBlast™ to fill the small gaps in the joints and for larger defects, he uses FlexiGRAFT® with Arthrex ACP® to fill the bony defect.

Subtalar Fusion with Biologics and the Arthrex®  Low Profile 6.7 mm Cannulated Lag Screw

All right. We're going to demonstrate a subtalar fusion and a utilization of appropriate biologics, as well as a fixation with the Arthrex cannulated screws. And there I'm seeing the peroneal tendons right there. So I'll make sure at this point there is no significant synovitis no spurs hitting the peroneal tendons. Make sure they're free. And then I work above them and coming above them, right above them, right there. And I am in the subtalar joint. What I want to do is provide biologic stimulation in that area. And there's different choices. And I keep all of these available in the operating room and open what I need. Stimublast putty has got very nice handling characteristics. It will fit nicely into tiny, tiny gaps or areas where we have nice congruency of the joint surfaces. So I'll often add some of that in and it will spread around where it needs to spread around. Now sometimes I will actually have bigger defects if I know I have areas where I have subchondral cysts or things that I want to fill more. I'll use the FlexiGRAFT fibers, you can use a saline to hydrate it. I like to use a platelet concentrate such as ACP, and once you place that in, you'll see that it will wick up nicely. And it makes these fibers very moldable. This will fit very nicely into bony defects. And so what I'll do if I'm looking at a bony defect, I will fill that in and pack it up against the bony defect. So what I will do is I will actually place the wire over the side of the foot and get a ladder, and I'll see where I want to start it. Then I take it through a percutaneous manner, place a guide pin down, and then I'll see if I'm sitting where I like. I want to aim towards the heel. I can get a very nice position. This is pretty parallel. I could easily get a second guide pin and screw in if I wanted to. So once I've got the position I like it. I will take generally an 11 blade and just make a little small nick here. I'll take a spreader and just kind of spread down along the wire and then I'll measure it. And this here measures approximately 85. Now I will use as many thread lengths as I can, just so that I think I can get away with. I'll put a guide down over. Now that guide will help protect any tissues around my percutaneous exposure. And I place the drill right on and I can drill pre drill the area. I'll now place on my selected screw. Now these are self-drilling self-tapping screws. So I didn't need to drill. That's a decision made on patient's bone quality. And I'll put it in. And when I get close I will stop and place on a hand driver. And I'll check an image to see how close I am to being seated. Very good fixation with appropriate biology in that Subtalar fusion site. So I would remove and I would get my final imaging as such.
