Thrombinator™ Device Hands-On Demo

Speaker  The thermometer device is a unique autologous biologic offering that differentiates Arthrex from its competitors. Derived from the patient's whole blood PRP, or PGP, the system harnesses the power of the clotting cascade to create a key element in creating a fibrin clot. The thrombin later eliminates the use of allogenic thrombin and does not use any harsh chemicals. The thrombin rich solution created with the thermometer device can be produced in less than 30 minutes and can be utilized in addition to a fibrinogen source of either whole blood, PRP or PGP as a powerful tool in a variety of procedures to deliver growth factors and improve handling characteristics. For this reason, it is vital to demonstrate the product and ensure end users gain familiarity with the steps required to produce a satisfactory result. One common challenge in preparing for this demonstration is collecting the necessary materials, including the type of blood needed and 10% calcium chloride. The video will outline everything needed to set up a proper demonstration, including where to source materials and instructions to follow in order to achieve the desired results. The thermometer kit comes with the thermometer system and an 18 micron filter. For this demonstration, you will also need Biofluid. This can be whole blood PRP or PGP calcium chloride deionized water a 20 to 30ml cup. This will hold the final product and show the clot. You'll also need 410ml syringes. Speaker  The first syringe will contain four millilitres of biofluid. For the first introduction into the thermometer. The second syringe will be for eight millilitres of biofluid. For the second introduction into the thermometer, the third syringe will be for the final thrombin serum output from the thrombin air device, and the fourth should contain additional biofluid, which will be mixed with the final thrombin serum output at a ratio of 1 to 1. You also need two one millilitre syringes. The first syringe will be the introduction of 0.1ml of 10% calcium chloride, and the second will be used for the introduction of 0.2ml of 10% calcium chloride. Finally, you will also need a food scale. This will be vital to properly mixing your calcium chloride solution to create a 10% solution of solid calcium chloride with deionized water for the bio fluid. In this demonstration, you will also need at least 20ml of bovine blood as anticoagulated with a CDA. Make sure to store it under refrigeration when not in use, then hand warm the blood prior to the demonstration. This will help to replicate normal clotting time and normal body temperature during the demo. Do not use Heparinised or Defibrinated blood, it will not clot appropriately. Blood can be sourced from groups such as Lampire Biological Laboratories. Lampire Biological Laboratories offers a wide variety of animal sources of blood. A pearl that we recommend is using bovine blood anticoagulated, specifically with acda. Speaker  The calcium chloride is needed to reverse the acda anticoagulant as acda acts to anticoagulate blood by quenching calcium. Laboratory or clinical grade 10% calcium chloride solution can be difficult to source and be expensive. However, you can formulate your own for demonstration use only. You must use high quality food or supplement grade calcium chloride, which should be free of impurities that can interfere with blood clotting processes in the thermometer device. When formulating your own 10% calcium chloride, it is important to control the calcium ion concentration in the final solution. For this reason, ensure you are sourcing laboratory grade deionized water. The food or supplement grade calcium chloride and the laboratory grade deionized water can both be found on Amazon. This will give you all the necessary elements for completing a thermometer device demonstration with just 0.3ml of 10% calcium chloride solution needed per clot. This recipe will enable you to run hundreds of demos when preparing for the demo. Mix the calcium chloride with deionized water to achieve the 10% formulation. You can do this by using a food scale with units set to grams. Add 90g of water, which is 90ml of water and ten grams of calcium chloride to a clean container and mix until the powder is fully dissolved. Using the first one millilitre syringe, aspirate 0.1ml of calcium chloride solution and connect the syringe to the inject port of the thermometer device by pushing down and turning to engage the swappable valve, then firmly and rapidly inject the solution. Speaker  One pearl here is to confirm splashing of the calcium chloride into the thermometer device. This will confirm that all calcium made it inside the device. Next, you will inject four millilitres of the bile fluid through the inject port. An advantage of adding the bile fluid after the calcium chloride solution is that the bile fluid will rinse any remaining fluid out of the swappable valve, disengage the syringe, and shake the device for five seconds. Once complete, place the thermometer device horizontally with the withdraw port facing up. Wait a minimum of 15 to 20 minutes before picking up or disturbing the device. During this time, the contents of the device should produce a clot. This time point is critical to evaluate during the O.R. workflow. Do not proceed with the next step of the thermometer workflow until you are ready to proceed. Due to the second step having a short waiting time. It is important to note that once the thermometer serum is removed from the device, it must be used within 15 minutes. If you do not need the thrombin serum quite yet, remain at this first step. The thermometer allows the first step to incubate for up to four hours, allowing for proper timing with the case workflow. Speaker  This device will produce a functional clot after wait times of up to four hours, provided you stay at this stage. This means that before beginning a demonstration, you can prepare a thermometer device in advance to reduce wait times and application of the output into the area of interest. When you are ready to use the serum, you can proceed by shaking the device for five seconds. Strike the thermometer device against the table or countertop to help break up the clot, then proceed by shaking the device for five seconds using your second clean one millilitre syringe at 0.2ml of calcium chloride solution into the thrombin device through the inject port like before, pushing firmly and rapidly on the syringe to expel all the solution. Then add the remaining eight millilitres of bile fluid into the thrombin device via the inject port. The bile fluid used in this step can be the same used in the first step or different. This means you can use either a combination of whole blood PRP or PvP between the first and second step. Mix the device for an additional five seconds, then lay the thrombin device on its side with the withdrawal port facing up. Wait 1 to 2 minutes for a second clot to form. Avoid picking up or disturbing the device during this time. A clot is swiftly produced within the device along with an excess of the protein thrombin. Speaker  After a 1 to 2 minute wait period, strike the device on the countertop or table to begin breaking up the clot and shake for another five seconds. Again, set the device down horizontally with the withdrawal port facing up for the third and final time, allowing the device to rest for one minute. The goal of this third rest time is to deplete all fibrin and create a thrombin rich output, which will reduce premature clot formation in the withdrawal syringe. After the one minute wait time is up, strike the device again and begin shaking for five more seconds. Even if the contents are not clotted, the device is now full of thrombin rich serum. Connect the external filter to the withdrawal port on the thrombin device. A pearl here is to twist the filter on by grasping the bottom portion of the filter. Generally, once the filter is engaged, you can hear a small hiss of air escape from the device, indicating that it has activated the swappable valve. If the filter becomes blocked at any point, try tapping the device to move the clot out of the way. Or you can place air into the empty ten millilitres syringe and press the air into the device, breaking up any clots that may be occurring around the withdrawal port. This will allow for better fluid flow out of the device. The filter is a critical step to remove all the small beads inside the device from the serum. Speaker  Connect a clean ten millilitres syringe to the other side of the filter, and invert the device at a 45 degree angle toward the withdrawal port to remove the serum. A typical thermometer will produce 5 to 7ml of thrombin serum. The extracted thrombin serum needs to be used within 15 minutes. Visually measure the output of serum you received from the thrombin device and ensure a 1 to 1 mixing ratio with your chosen biofluid. You can inject this directly into the 20 to 30ml cup to visualize the clot or area of interest in a cadaveric specimen, like a filled auto kart defect. Let the mixture rest for 30s as it sets into the final clot. Testing of our homemade formulation against a Preformulated laboratory grade solution on platelet poor plasma showed equivalent results. Clotting occurred at a similar time interval in less than 15 minutes. Clotting time stated in the directions for use. The clot showed similar strength and consistency. This indicates that the homemade version is a sufficient alternative for demonstration use only. You can follow these steps to make a clot with the thermometer device, showcasing the system's ability to harness the clotting cascade and autologous blood products without allogenic thrombin or harsh chemicals. Demonstrating the product also ensures that the end user gains familiarity with the steps in a non-critical setting.
