Elbow Arthroscopy Using the NanoNeedle™ Scope 2.0

Speaker  Hello, I'm Doctor John from Miami, Florida. And today I'll be demonstrating the updated nano elbow arthroscopy technique using the nano needle scope 2.0. We've already created our medial and lateral portals. As you can see here. This is the anterior radial portal and the anterior ulnar portal. For those of you that are interested in knowing all of the arthroscopy portals for the elbow, there is a video on the Arthrex website that demonstrates all of the portals for elbow arthroscopy. As I've noted, we've already created these medial and lateral portals, and I'll walk you through the placement of high flow sheath and how it enhances visualization and fluid management. As you can see, the working portals are already in and the high flow sheath where the camera has the fluid attached to it. Let's move inside the joint to observe the enhanced image quality provided by the Nanoneedle scope 2.0. Here's the initial view from the radial portal. Notice the clarity and depth of field, an improvement over our previous generations of this scope. You can see the Trochlea and coronoid as you bring it through range of motion. Again, the Nanoneedle allows for excellent maneuverability and we can see the radial ulnar joint even from the radial portal and the radial head and portion of the capitellum. Now I'm going to switch to the ulnar portal. And you can see it's easy to move between the working portals with the high flow sheath on the camera. Now, from the ulnar portal, we continue to see crisp visualization, which aids in precise diagnosis and treatment during shaving. Speaker  The improved optics allow for better tissue differentiation and safer debridement. Here you can see the annular ligament very nicely. Radial head capitellum. And you can see a beautiful view all the way back to posterior aspect of the elbow during shaving. The improved optics allow for better tissue differentiation and safer debridement. So here we're using the SX 50 radiofrequency wand. Take out the working cannula for this and place this in percutaneously. And this is an excellent tool for chondral defects throughout the elbow. You can use it posteriorly in the trochlear groove, which will show in a variety of other areas where it becomes very valuable in doing chondroplasty in the elbow. So reviewing from our posterior portal. Now, as you can see, and we're able to see the radial head beautifully. Anterior to posterior. Medial to lateral. Here's your capitellum. You can see an excellent rotation. And again you can see the significant difference in the quality of the picture in the new nano 2.0. As we work our way up the trochlear groove, you can see all the way across the trochlea and olecranon. As we work our way more posteriorly, we can work our way all the way back into the posterior chamber of the elbow. You can see a beautiful view here of the again olecranon and trochlea. If you have loose bodies or other synovitis, you make an accessory portal. Again, you can see this on our other videos available arthroscopy. And in doing so you gain an excellent view and get instrumentation, whether it's a grasper, a shaver in this area. Speaker  As we come more posteriorly again through the working cannula, you can put our high flow sheath. Here we are in the very posterior aspect of the elbow. We're looking at the olecranon fossa region. Arthrex has created two millimeter graspers that work beautifully in removing loose bodies. They can fit right through your working cannula to grab loose bodies and whatever area you happen to be in, whether it's posteriorly anteriorly down the gutters. This is an excellent grasper for these type of areas that you need to get a small instrument into. We're looking right at the olecranon fossa. There's the tip of the olecranon. You can see if you're flexing and extending the elbow, you can make sure that you're not impinging anything in the fossa itself. And as we come down to the medial side of the elbow, you can see a beautiful view of your ulnar collateral ligament. There is the transverse bands. You can see the anterior and posterior bands. And this again is a very common place for loose bodies. I usually like to try to milk this just to make sure there's nothing else down there. But this is an excellent view that the Nanoneedle gives you. Dramatic improvement over any other type of arthroscopic tool we have today. To summarize, the integration of the high flow sheath with the working Cannulas significantly improves fluid management and procedural efficiency. Combined with the enhanced imaging capabilities of the Nanoneedle scope 2.0, this technique offers a powerful tool for elbow arthroscopy, especially in tight joint spaces. Thank you for watching.
