Low Profile Locking Proximal Opening Wedge Osteotomy Plate

The Arthrex low profile titanium locking proximal opening wedge plate is indicated for patients presenting with mild to moderate valgus deformity. In instances with a more severe deformity, the locking POW plate can be used in conjunction with a distal metatarsal osteotomy. Each millimeter of wedge typically accounts for three degrees of valgus deformity correction. An initial excision of the lateral soft tissue structures is carried out. Resection of the medial eminence is completed using a sagittal saw. An osteotomy is created at the first metatarsal, approximately 1.5cm distal to the first TMT joint. Slowly distract open the osteotomy site using the small osteotomies provided in the comprehensive foot set. Once an appropriate space has been has been created. Insert the mini joint distractor on the plantar aspect of the osteotomy site and slowly distract. Open the osteotomy to the desired correction with careful attention taken to prevent violation of the lateral metatarsal cortex. Insert the metatarsal opening wedge trial instrument to determine the appropriate sized wedge. After determining the appropriate locking wedge plate, provisionally attach the plate to the bone using B-B tacks. Attach the 3.0mm locking drill sleeve to the proximal planter screw hole in the plate, and drill bicortical through the proximal phalanx. The appropriate length screw can be confirmed using the laser mark on the drill guide, or by using a standard depth device following removal of the locking drill guide. Alternatively, a 3.0mm variable angle locking drill guide can be used if desired. Insert a 3.0mm millimeter, locking screw into the plate and advance until the screw is flush with the plate. Remove the proximal dorsal BB-Tak. Attach the 3.0mm locking drill sleeve, and drill bicortical in a similar fashion. Determine the appropriate length. Screw and insert another 3.0mm locking screw into the plate until it is seated flush with the plate. Using the appropriate drill guide, drill an eccentric drill tunnel bicortical through the metatarsal. Using a standard depth device, determine the appropriate length. Screw and insert a 3.0mm cortical screw through the plate. Finally, remove the distal tack and attach the 3.0mm locking drill sleeve. Drill bicortical. Note the appropriate depth and insert the final 3.0mm locking screw. This completes fixation of the construct. The Arthrex locking proximal opening wedge plating system provides a systematic and graduated approach to treat valgus deformity. This technique also allows surgeons the option to perform a distal metatarsal osteotomy for those patients presenting with a more severe valgus deformity. After achieving final fixation, fill in the bone void created from the opening wedge osteotomy using one cc to 2.5 cc's of Arthrex StimuBlast demineralized bone matrix.
