Das iBalance® HTO-System besteht aus nicht resorbierbaren Implantaten und Ankern aus Polyetheretherketon (PEEK), die zur Fixierung und Aufrechterhaltung der medial öffnenden proximalen tibialen Osteotomie während einer HTO-Versorgung in den Osteotomiespalt eingebracht werden. Das System bietet eine alternative Option zu traditionell verwendeten Platten und Schrauben.Die iBalance HTO-Implantate und -Anker verbleiben im Knochen, wodurch eine Zweitoperation zur Materialentfernung, wie sie in manchen Fällen aufgrund von Weichteilgewebereizungen über einer Platte notwendig ist, verhindert wird.
Um die Heilung zu beschleunigen, der Rekonstruktion zusätzliche Festigkeit zu verleihen und möglicherweise eine schnellere Belastung zu erreichen, können Biologika wie OSferion-Knochenersatz für Opening Wedge-Osteotomie (AR-13370-01-04), Arthrex® Quickset™, StimuBlast®, demineralisierte Knochenmatrix und Autologes Conditioniertes Plasma (Arthrex ACP®) verwendet werden.

Procedure

Die iBalance® HTO-Operationstechnik beginnt mit einem speziell für die iBalance Implantate entwickelten Instrumentenset mit Schutzhaken, die eine sichere „Hülle“ bilden, sodass alle Schnitte sicher und reproduzierbar ausführbar sind und die Gefahr von Nerven- und Gefäßverletzungen sowie Frakturen des lateralen Bands erheblich reduziert wird. Die Instrumente ermöglichen darüber hinaus eine Ausrichtung der Osteotomie in sagittaler und frontaler Ebene, sodass die Tibianeigung erhalten bleibt. Die schrittweise angeleitete Technik des iBalance HTO-Systems erlaubt dem Operateur einen sicheren und reproduzierbaren Eingriff. Das iBalance HTO-System ist insbesondere für Operateure geeignet, die eine hohe tibiale Osteotomie durchführen möchten.

Nach der präoperativen Berechnung des Korrekturwinkels wird die mediale Tibia freipräpariert und das ventrale, mediale und posteriore Weichteilgewebe vorsichtig abgelöst. Nun erfolgt die tibiale Osteotomie durch Platzieren der Arthrex HTO-Schnittlehre an der medialen Tibia und Einführen von zwei 2.4 mm Führungsdrähten bis ca. 1 cm tief in die laterale Kortikalis unter intraoperativem Röntgen. Der Operateur kann auch die biplanare Ausrichtlehre von Arthrex verwenden, um die Höhe der Osteotomie zu bestimmen und sie an den natürlichen tibialen Slope des Patienten anzupassen. Nach Platzierung der gewünschten Arthrex Schnittlehre und der Retraktoren erfolgt die Osteotomie mithilfe eines Osteotoms oder eines speziellen Sägeblatts. Der Osteotomiespalt wird mit dem Arthrex iBalance HTO Opening Jack, Osteotomy Jack oder Osteotomiekeil bis zum gewünschten Korrekturwinkel aufgespreizt.
