CuffMend™ Augmentation With the SpeedFLEX™ Implant for a Partial Articular-Sided Tear

Speaker  I'm Rafi Mirzaian from Los Angeles, and I'll be demonstrating a CuffMend repair of a partial articular sided rotator cuff tear. We'll be using the CuffMend implant system as well as a SpeedFLEX implant for this procedure. Now that we've unboxed our CuffMend implant system, we can find a portal skid, a mega loader, a tensioner, cutter, graft inserter and spreader two Self-punching peak tipped PushLock anchors as well as FiberStitch RC curved. We will now open the SpeedFLEX implant. We will now remove the SpeedFLEX implant. And as you'll see, the sutures are well cleated to prevent any entanglement. Please note that laterally the SutureTape TigerLink are not luggage tag, which gives you versatility for lateral fixation. We will now load the SpeedFLEX implant onto the graft spreader. We've just finished loading the SpeedFLEX implant onto the graft spreader. I'd like to note that there's four options for the SpeedFLEX graft, which is one and two millimetres grafts. There's a 20 by 25 millimeter option and a 25 by 30 millimeter option. So we're looking at a right shoulder. I've established an anterior portal. And what I like to do is use a spinal needle to just enter posterior to the biceps tendon and medial to the rotator cuff cable and place a blue prolene suture through the spinal needle. And I grasp it from the anterior cannula and pull it out. What this will allow me to do is once I'm in the subacromial space, it will identify the anteromedial boundary over the SpeedFLEX implant will be. Speaker  Here. I have the SpeedFLEX implant that I'm going to introduce from the lateral portal. So this is our first FiberStitch RC that's being placed in the Anteromedial position. It goes right through the SpeedFLEX. Clearcut back and forward. And note that I start with the border of the implant first. And you want to be careful not to pull it out so you don't develop a tissue bridge. And then the second pass is made three millimetres or so posterior to it. All three suture limbs are then grasped and pulled to seat the pledget. So you can either use a probe or with your fingers, as long as you're tightening that loop. Just like that. Perfect. And then once you've done that, you can pull the free limb and I'll tie it down. The suture is in place through the tensioner cutter that comes part of the kit. And we want to back up about two millimeters or so. Before cutting this, grab all three suture limbs together to set the pledget so the prolene suture can now be removed. At this time, we're going to undo the sutures that are on the cleats of the inserter. What I like to do is cut one of the limbs really short by the cannula before pulling the other one out. That shortens the length of the suture that has to go through the graft and around the graft to come out. And now the SpeedFLEX inserter can be closed and taken out. Speaker  So this is why this posterolateral portal is important, because now I'm going to go in and grab the luggage tag and pull it out of this portal. So now the graft is nicely spread out over my rotator cuff. What I'd like to do next is mark the site of my self-punching PushLock through the tissue, so that it makes it easier for me to see. We're now loading the luggage tag into one of the self-punching push locks, and that is slid down through the passport cannula. And once we're down, we don't want to put the SpeedFLEX under a lot of tension. So what I do here is if this is where I'm going to enter, I pass point. So when I come back up there's plenty of slack left in the suture. So create a big loop of suture here and find where I'm going to insert. Go ahead and mount that in. There we go. And tap that in. So by removing that orange hub, you see that the graph is not being further pulled into the anchor. We're just fixating it as it lays. We're now going to fixate our anterolateral. If you want to, you can further tension this by pulling on the suture just like that. Okay. Go ahead and take the orange hub off. We're now viewing our final repair with the SpeedFLEX implant of our partial articular sided repair from the posterolateral portal.
