The procedure highlights a double-row repair using a Knotless 2.6 FiberTak RC soft anchor medially and a 4.75 mm Self- Punching SwiveLock® anchor laterally, with the addition of an embroidered polyester FiberButton to improve compression at the tendon-to-bone interface.

FiberTak® SpeedFix™ Rotator Cuff Repair With FiberButton Medial Compression

Speaker  Tal David from San Diego, excited to share with you a technique for rotator cuff tears called Fiber Tack Speed Fix with fiber button. This technique involves a handful of implants that you're already familiar with. We have the 4.75 self punching swivel lock for our lateral row. We have the 2.6mm fiber tack soft anchor for a medial row. And the unique thing about this technique is the addition of a fiber button, which allows us to compress our medial interface between the tendon and the bone, and adds an additional security to this repair by broadening the compression between tendon and bone. This fiber button is unique in that it's woven from polyester. The pre-punched holes are approximately six millimeters. Okay, so here's our shoulder. It's a right shoulder. We're viewing it from our standard posterior viewing portal. Okay, now we've come through Percutaneously with our Inserter. In this case, we're going to drill a hole for our implant. You can also self punch. These are self punching Inserters usually insert these by hand until you feel some resistance and then have your assistant mallet it down. There's a nice broad laser line at the top of the handle, and we're going to watch that come down. That way we know how far to go. That's good enough there. Anywhere in that black zone is the right place to stop. Take our ordering off and simply back out the inserter. Our medial implants embedded in the bone. Speaker  We're going to pull on all of our strands just to set the anchor. I just want to go through the sutures so you understand what you have here. You have the two black fiber tapes. These fiber tapes are swedged at the end. So for this technique, before we before we proceed, we'll cut the swedge. So we have two independent sutures. Those two sutures will allow us to pass each tape through a different point. The other sutures that we have here you're familiar with, you have the repair suture. The repair suture has the black and blue stripes, and then you have your shuttling suture. This shuttling suture goes through the core of the implant and will allow us to use that knotless mechanism immediately. Our next step will be to deliver the sutures that we want to pass. So this tape retriever allows us to come through and pick out the sutures that we want, because we'll do an anterior pass first. But take one tape with your repair suture and these will be passed together. Next we use a scorpion with a fiberlink. And this is essentially the way we're going to pass our sutures medially. So I'll pass the fiber link. So we're going to take our repair suture. And one of the tapes we're going to pass it through the loop of the fiberlink. Again this is just a way for us to shuttle these two sutures through the front. Speaker  So now we have our tapes and our repair suture passing anteriorly and medially on our rotator cuff. So our link is brought back in an independent pass, and our button has two holes that are six millimetres apart, so I don't want to pass it too close to my original passage. I want to give myself some space and I think this is a good spot. We'll pass the link here. Now I have my link and I have my tape and my shuttle sutures. I'll pass those through my accessory portal. And now what I've accomplished is I have a tape anterior and posterior. I have the repair suture anteriorly and I have the shuttle suturing posteriorly. Now I'm ready to fixate my tapes laterally. I'll take the anterior tape. Take the posterior tape. I'm going to pass those through that self punching swivel lock. And we're ready to create our lateral row. So now we've accomplished our rotator cuff tension. Now we're going to introduce our button and use that medial knotless construct to create medial compression of this repair. So our next step is going to be to deliver the repair suture through our passport and the loop end of the shuttle suture, which is the rounder of the two sutures. We're going to add the fiber button to our medial construct. And the fiber button will come loaded with our flag. Speaker  Makes it very easy to pass the suture. We're going to take our repair suture. We're going to pass it under over and then back down across the button so that we have a horizontal construct using these two pre-punched holes. So now the suture has been loaded through the holes in our the pre-punched holes in our button. And we're going to convert the repair suture. But one thing you want to make sure you do is leave yourself plenty of length so that you don't pull the button into your construct. You want to leave the button external, and then we'll deliver it manually. It just creates a much easier flow, creates less bulk inside the subacromial space. Now we're ready to convert our repair suture medially. If we left the button out here, we've converted our anchor. So I just want to have really good control as I'm delivering this button into the subacromial space. I like to use a Fiberwire grasper on the button. I don't want to grab suture material, so it doesn't matter if you want to orient it transversely. If you want to orient it horizontally, it might be easier to deliver it through your passport this way. But make sure that you're not grabbing sutures so that in the event that you're pulling suture through this construct as you're tensioning the loop, you're, you're not impeding the ability of that suture to flow through the holes. Speaker  We're ready to deliver our button. I'm making sure to help the button down, just because I don't want to create a tremendous amount of tension in this. So I'll deliver my button. I have my assistant carefully tensioning that medial row and watch the thing as it comes down. And then at some point here I'm going to want to release the buttons. Now, if I want to fine tune it, I can use my probe. You still have a lot of ability to move and mobilize this. You can you can pull your sutures through. You can use the probe through the hole to help orient your button one way or the other. So I like I like the way this is coming down. I've got good control over it. And then I'm going to terminally tension my loop. You can see how secure this button is. Excellent compression. This really gives me the ability to seal that medial edge. A lot of times we worry about synovial fluid tracking under the articular side of this tear and disrupting the healing that's going to occur laterally. So this really gives us a great ability to compress that and also provides us with a bit of a rip stop for our sutures. Very secure, perfect size for this size rotator cuff tear. And we've got a rotator cuff tear that's got medial compression double row repair lateral fixation.
