SpeedScap™ Subscapularis Repair After Eclipse™ Total Shoulder Arthroplasty

Speaker Performing an anatomic arthroplasty and just finished the glenoid component. And now I'm going to move on to the subscap repair. I'm going to be using the SpeedScap system. This comes with a trunnion guide to allow us to avoid the central cage screw, which has been placed into the cut surface of the humerus. When using this guide, it's critical to understand the way it works. This is sized for a small cage screw, which we've placed in our patient. I'd like to be perpendicular with my drill. This allows me to be sure that I'm going to avoid the underlying screw. So I'm going to drill for our first FiberTak anchor. That drill has a positive stop can then simply take the anchor, which has a more robust handle for open instrumentation. Place this into the repair site where the Subscap will go. This essentially serves as our medial row for a double row Subscap repair. Can see the bed of the wafer LTL that I performed and I'm going to avoid that bed. If you're doing a peel. This can be placed directly into your peel release site, going just around that. And then I can drill again perpendicular to the guide. You can see those hash marks above. Those help us to avoid the feet on the bottom of the trunnion itself. Once that second FiberTak is set, I'm going to drill for my third FiberTak below, and then we have our three FiberTaks ready for repair. Once this is placed, then I can remove the trunnion guide for the Eclipse cage screw. Speaker I like to use this guide where it exits laterally. You can see the bottom aspect of the guide is actually a perfect spot for placing those SwiveLock anchors. The next step in a subscap repair for me is. I like to set the upper border of the tendon. I use a 1.3mm SutureTape. To do this, I have my assistant hold the tendon through our traction sutures and the proper position, and then I'm simply going to take a SutureTape and repair the leading edge of the supraspinatus to the upper portion of the subscap. This gives the tendon for me a anatomic position, so that I'm sure that I'm putting it back where it's supposed to go. I don't tie this. We'll simply tie this later. Next, I'm going to use our free needle to place the sutures around the subscap. This can be done with the free needle that comes in the SpeedScap repair kit. Or we can utilize a Scorpion with a link for passage. I'm placing all of my sutures through. Three passes to allow for this double row repair. So I pass this around my Elto four. Utilizing a link here to pass this through. So this is another way that you can shuttle all those sutures. So we can see that the bony reduction is very important for me. I'd like to get that right so that I can get an osseous union. And we can see that on X-ray. So then I'll tie the upper border sutures that were previously placed, leaving about a one centimeter tail to. Speaker Repair here. Then we'll take one of each of the tapes that are medial to that Elto fleck. And we'll bring this over to a 3.9mm SwiveLock laterally. These anchors have both the tape as well as a FiberWire that can be tied later for medial compression, if you wish. Sometimes there are some remaining soft tissue in the biceps groove. The biceps groove has very nice bone for lateral row anchors. We'll perform a dissection. This drilling can be done before or now, as sometimes I will do this directly off the lateral portion of that trunnion guide before as well. So now we can place our anchors into the biceps groove. I'll use a freer to make sure it's down all the way. Sometimes it's difficult to see. You can see the head is up just a little bit more. So I'll take that down a couple more turns. We'll place our second anchor just below that to allow good spread at least a centimeter or so between the anchors laterally. Because we have a small screw, we're certainly lateral enough here. If you have a larger screw, it's wise to use the trunnion guide for making sure you're avoiding that screw before placing the SwiveLock anchors. So the medial row can now be tied. I'll tie the unlike to unlike sutures and then do the same to get medial compression across our repair. And so we can see a very nice compressed double row repair for this anatomic shoulder replacement.
