Speaker  Hi, I'm Doctor Michael Gorlitsky from the Steadman Clinic in Vail, Colorado. I wanted to speak to you today about an interesting case about SI joint pain and how we treat that using the Transection approach for the lateral branches of S1, S2 and S3. So today I'd like to talk about a 38 year old female who had chronic SI joint pain again. This patient had multiple rounds of radiofrequency ablation, but it just didn't last. And the biggest issue with that is there's a lot of branches that course out of the S1, S2 and S3 neuroforamen that bifurcate and come out towards that SI joint. And the difficulty with just putting a single RFA probe is you're going to miss some of those branches. And in this patient, we're able to do a unilateral right sided S1, S2 and S3 Transection. And what we do is we start at the 12:00 and 6:00 position of S1, S2, S3. We make sure we get good X-rays, both on AP and Lateral to make sure that we're on top of bone, as the sacrum itself is very undulating. So it's not necessarily a flat bone like the previous lumbar or cervical medial branch transactions, which typically have a flat landing zone. Speaker  So in the sacrum we need to go through a lot more ligaments. And once we get down to the bone itself, we need to identify from the 12:00 to the 6:00 position, all the little lateral branches that start to course out. Once we identify these, we under direct visualization, snip them with either a snipper or any tool that will transect that branch. And then we finish that off with Electrocautery, making sure instead of just doing a point like the RFA does, we want to do a strip basically from the 12:00 position all the way to the 6:00 position, making sure that we do not go medial into those neuroforamen. Now, this patient, after the procedure, had a good outcome, their SI joint pain improved, and at this point they are six months out with no recurrence of their pain. And my protocol for this is just a nylon stitch to close those three separate poke holes. Once those are closed and the skin is healed, I let them return to activity as tolerated.
