Arthroscopic Peripheral Capsular Repair of the TFCC Using the NanoNeedle™ Scope 2.0

Speaker  Hi, Sanjay Kakar at Rochester, Minnesota. And today we're going to be showing the over the top tfrc technique for peripheral capsular tears. Okay, so here we have the camera coming in the six portal, and we have the short gray nano working cannula coming through the three four. And so here this patient has a peripheral repair, not a foveal repair. Same technique. The only difference is that we're coming through the periphery of the tfcc. And we're going to grab the capsule. So we're not going to go through the foveal insertion of the tfcc. So here's you can see the short gray nano working cannula. And again, the key part of this is taking the orange handle, a micro suture lasso, going straight down the middle of this, and it should be an unimpeded view. And then I see the tip and we're coming over the top now doing a peripheral capsular repair. So here we are. Here's the needle. If you're not sure of terms of your anatomy, here's bone. Okay. You can use this as a tactile probe. Now we'll use this. And you can see it's much softer. So again I want to take a wide bite of tissue. So rather than with a foveal repair I'm really lifting up my hand here. Speaker  Here I'm actually lifting it up just to go. And we're going to come underneath Tfrc and we're going to get a bite here, but we're not going to come down to bone. So this will actually you can use power, but typically you can pass this just free. So we're passing it through and you can see how it just passes through because this is a capsular repair. Again this is not a foveal repair. So once we do that we take this out. We take a 0.9mm suture tape. So because we're not actually drilling through bone, this is actually quicker than the foveal repair, believe it or not. So we'll just pull this through. This comes through your ulnar incision. And as we pull this through, we'll just dunk this in the cleat again. Take the orange handle. And then come through the middle of the nano cannula again unimpeded. See my tip? There I am. Now let's say you wanted to do a foveal repair. Very simple. You just lift up your hand and come directly onto bone and lift your hand up. But because this is a capsular repair, there's the one bite of it. I'm coming over. I'm walking directly. I'm feeling capsule. I'm right on capsule. Speaker  And again, we can just push this through. It should go unimpeded. And this will just come straight through the capsule, as you can see right there. And then we'll take the other limb of the tape. Take this out of the cleat. And we'll pull this through, just making sure it doesn't get caught in the cleat. There it goes. Passes seamlessly through. No suture bridge. And there's your peripheral repair right there. And that will give you your your suture configuration. And you can simply use a two zero fiberwire if you want and pass another suture here or another two zero fiberwire and pass another suture here. Okay. So depending on your tear we pass one suture here. You can pass 2 or 3. You can again critically you have to be out of traction. And you can do a push lock to bone. Or you can simply tie this down to capsule to give your capsular repair. So here we are. The nerve is retracted out of harm's way. Pull down. Just just relax. Once we're down, relax. Take the creep out of the soft tissue and then come down. Put a sliding, not straight down. So here you can see the low profile capsular repair of the triangular fibrocartilage complex.
