Endoscopic Sacroiliac Joint Denervation

Speaker  The Arthrex endoscopic approach for sacroiliac joint denervation uses an endoscope to provide access and direct visualization of the anatomy. The patient is positioned prone. A C-arm is used to obtain an orthogonal AP fluoroscopic view of the sacrum to target the junction of the S1 superior articular process sap and sacral Ala junction. Insert the introducer needle using AP and lateral views to guide the needle to the S1 sap and sacral Ala junction. Remove the stylet and pass a guide wire through the cannula. Make an incision through the skin and fascia to accommodate the outer diameter of the dilators and working cannula. A switching stick is inserted over the guide wire, followed by placement of the cannula. The opening of the cannula is oriented cranial and medial. The cannula holder is securely attached to the TRIMANO arm adapter, helping to maintain the desired trajectory. A depth stop is attached to the endoscope to maintain appropriate depth within the cannula. The switching stick is removed and the endoscope is inserted. A series of endoscopic instruments, including an Electrosurgical probe, Graspers and additional tools are introduced through the working channel of the endoscope to remove excess tissue and coagulate blood vessels. Arthrex offers a unique whisker shaver that aids in the efficient removal of soft tissue. Once the L5 medial branch nerve is clearly visualized, it is transected. Using a punch or hook scissors, use the Electrosurgical probe to treat the nerve endings and debride the bony surface progressing upward toward the facet joint. Speaker  Translate the endoscope and cannula caudally and laterally to visualize the S1 lateral branch. Transect the nerve using a punch or hook scissors. Using an orthogonal AP view target lateral to the S1 and S2 foramina, perform the same needle guide wire incision, dilation, and cannula docking steps used for the initial access. Orienting the bevel. Caudal medial. Attach the cannula holder, then introduce the endoscope to continue the procedure. A series of endoscopic instruments, including an electrosurgical probe, graspers and additional tools, are introduced through the working channel of the endoscope to remove excess tissue and coagulate blood vessels. Once the S2 lateral branch nerve is clearly visualized, it is transected. Using a punch or hook scissors, use the Electrosurgical probe to treat the nerve endings and debride the bony surface. Careful to remain lateral to the S2 foramen, translate the endoscope and cannula caudally and laterally to visualize the S3 lateral branch. Transect the nerve using a punch or hook scissors, use the Electrosurgical probe to treat the nerve endings and debride the bony surface. Careful to remain lateral to the S3 foramen. Remove the endoscope and cannula. Apply JumpStart antimicrobial wound dressing over the incision site.
