Onlay MPFL Reconstruction With Hybrid Knee FiberTak® Anchors and FiberTag® TightRope® II PF Implant

Mpfl reconstruction with patellar onlay fixation using NE fiber anchors and femoral fixation with the fiber tag. Tightrope two implant. The Onlay Mpfl kit features all instrumentation, implants, and sutures to perform an mpfl reconstruction, including two hybrid NE fiber tack anchors on tapered non self punching inserters for easy anchor implantation, a NE fiber tack ratcheting guide to stabilize the patella while drilling the pilot hole and implanting the anchor, and a fiber tack. Tightrope two implant for precise tension. Femoral fixation. To begin the technique, make a two centimetres skin incision from the superomedial corner, extending to the center of the medial edge of the patella. Create a bleeding bone bed on the medial patellar edge. Using a rongeur or powered burr, place the spike of the NE fiber tack ratcheting guide firmly on the lateral rim of the patella, and slide the ratcheting drill sleeve down to the medial rim of the patella. Create a pilot hole for the anchor by advancing the 2.8mm hard bony fiber. Tack tech drill until it bottoms out against the back of the drill sleeve. Insert the hybrid knee fiber anchor by hand, advancing the tip of the anchor until it is within the pilot hole. Using a mallet, gently advance the inserter until the inserter handle bottoms out against the back of the drill sleeve. Remove the anchor inserter and ratcheting guide. Gently pull the black suture limbs to set the anchor in the patella. Repeat the drilling, insertion, and setting process to place a second anchor, spaced approximately 15mm to 20mm from the first anchor. Pass the prepared graft through the blue knotless loops of both anchors. Tension the graft down to the medial patella by pulling the blue tensioning strands. If using the fluoroscopic template, place the template on the area of the medial epicondyle on the distal femur. Under Fluoroscopic guidance, a four millimetres spade tipped tightrope pin is targeted at shuttles point and is drilled across the femur. The pin should be aimed slightly proximal and anterior to avoid the intercondylar notch before reaming a socket, the isometry of the mpfl may be provisionally evaluated. The graft limbs are marked at the appropriate length with adequate tension. Use a six millimetres low profile reamer to create a socket. Maintain the tightrope drill pin in the femur as it will be used to pass the graft into the femur. Identify a point on the graft approximately three centimetres past the isometric length. Markings and trim the graft limbs, leaving two limbs of equal length. The graft limbs are trimmed to the appropriate length based on the reamed socket depth, with the fiber tag suture pierced onto the tooth of the graft clamp clasp approximately two millimetres from the end of the graft, limbs being careful to ensure the tines are engaging both limbs. Determine the position of the fiber tag. Suture. This pass should occur where the fiber tag converts to fiber loop suture. Perform the standard speed whip ripstop technique, working toward the tightrope two suture card and ensuring the fiber tag suture is captured with each needle pass after placing two speed whip stitches in the graft. Pass the needle through the slot in the suture card, ensuring the needle passes over the tightrope implant. Repeat the speed whip ripstop technique with two additional passes working away from the graft clamp instrument, ensuring that the fibre tack, suture and both graft limbs are captured with each pass. Cut one limb of suture just below the splice of the needle. Wrap the suture limbs around the graft and tie the knot to secure the construct. Cut the suture limb without the needle just above the knot. Pierce the needle through the graft on the knot side. Pull the needle and suture completely through the graft and apply tension to bury the knot in the graft. Cut the suture limb flush to the graft. Remove the fibre tag tightrope suture card from the graft clamp instrument. Unwrap the sutures from the suture card cleat and remove the tightrope implant loops from the retaining slots in the card. Using a curved clamp, pass a fibre snare suture, leaving the looped end out of the patellar incision. Load the blue passing sutures and white tensioning tails through the loop of the fibre. Snare suture, and pass the sutures from the patellar incision out the medial femoral incision. The loops of the tightrope can be lengthened to ensure ease of passage and button deployment. Shuttle the single tail end of the fiber. Snare suture using the eyelet of the tightrope pin and deliver out of the lateral femur. Load the blue passing sutures in the loop of the fiber. Snare suture and deliver them out of the lateral femur. Pull the blue tightrope passing suture from the lateral side to deliver the button through the lateral femoral cortex, using a curved hemostat under both graft limbs as a pulley to manage tension. Final fixation can be accomplished by pulling the tightrope implants, tightening sutures on the medial side, alternating tension on each strand. The black 1.3mm sliding suture tape. Limbs with swaged needles from the patellar anchors provide added versatility, and can be used to close the retinaculum passed through the graft for additional fixation, or to accomplish a VMO advancement as needed.
