The Knee Obturator has been specially designed to simplify the location and creation of posterior knee portals, as well as introduction of a cannula. Posterior portals are often necessary for several common procedures such as loose body removal, PCL reconstruction, meniscal root avulsion repair and popliteal cyst debridement. The knee obturator saves time and adds a measure of safety to the procedure when compared to traditional outside-in techniques with a spinal needle. The knee obturator also allows easy insertion of the Passport Button Cannula utilizing traction sutures. Once inserted, the Passport Cannula facilitates easy passage of straight and curved instruments or an arthroscope into the posterior compartment.

Simplified Posterior Portal Creation and Cannula Insertion Using the Knee Obturator

Today we're going to demonstrate the use of the knee obturator for posterior portals. In this particular instance the patient is a PCL deficient. We want to establish a posterior medial portal so that we can have access to the posterior aspect of the knee. Through that portal, we can often debris the PCL stump, establish our tibial tunnel or socket. And so posterior medial portal is very advantageous for that. So with the knee obturator posterior portal device we have a shape that's a little serpentine. It's designed to get around the femoral condyle posteriorly. If you look at it closely, curve that's here in the middle is there to accommodate the ACL so that it goes around the ACL without putting pressure on it. And then at the tip of the device is a is an opening which we can use to pull things into the joint, such as the spool cannulas or passport cannulas or other devices from the posterior lateral or posterior medial side. We'll start with a rigid cannula. Again, you can see how the curve is used to help accommodate the the ACL as we look posteriorly. We can get an idea of a target that we'd like the cannula to to enter the joint. And it gives us the best angle at our at our fossa posteriorly and then with some pressure. And I often will take the camera out once I started my pressure. That way I can actually feel the end of the device. And have it Pop through so that it now is tinting the skin. Just make a knife wound right over that. I typically then would just take the cannula. It doesn't need a obturator of its own. We'll take the rigid one first and just slide it basically right over the cannula. And then work both together with these little bigger rigid ones. Sometimes it helps to just turn them as they go in. So now with the establishment of our Posteromedial portal, we can back up our canyon a little bit. And now we can use that in a number of ways to help facilitate the procedure. We have access now to all of the stump of the PCL, so we can clean all of this off so that we have access to the back shelf to make sure that we get down far enough before putting our guide in to establish our PCL tunnel. This is a This portal is also useful, as you can see for route Avulsions. The corner of the piece of the meniscus. The medial meniscus, the posterior horn is right here, so it can be advantageous to help with route avulsions and route avulsion repairs. This also can be used as a viewing portal. [00:03:36] So all right. So now we're looking directly at the stump and back towards the front of the knee joint. So up and over. And as we clean off our stump here, we'd be able to see right down the back of the joint. So that way you can see anything that you do, such as drilling guide pin towards the back. It's all under direct visualization. And now we're going to demonstrate how we use a passport cannula. So we're going to pass a suture Through the passport cannula. [00:04:20] That's correct. Doctor? Yes. I want to put this together. So, you see, there's a there's a crisscross stitch through the base of the cannula. We're going to pass the sutures through the obturator opening. [00:04:43] Try forward to see the safety to the grenade, you know. Do you want me to do that? So now what we want to do is pass these sutures back out the anterior portal. The sutures are now all through the front, and then with some resistance from the other side, allowing me to win. [00:05:22] There we go. You have to kind of pull back and let your let your suture hand win. But moving it forward. Pull the suture tails out. So now we've established a portal again poster medially with the passport again giving us access to the PCL fossa. The nice seal posterior medially and no water extravasation into the sub tissues. Subcutaneous tissues.
