Using standard wrist arthroscopy portals and setup, type 1B tears of the TFCC are repaired back to their anatomic insertion on the bone. Using a specialized Micro SutureLasso™ suture passer, a 2-0 FiberWire® suture is passed through the TFCC and a mattress stitch is created. The TFCC kit is then used to place a knotless 2.5 mm PushLock anchor into the base of the ulna. The sutures are tightened, bringing both the superficial and deep fibers of the TFCC back to their insertion site on the bone. The knotless anchor is inserted and final fixation of the TFCC back to the bone is achieved.

The Micro SutureLasso™&nbsp;replaces 18-gauge needles for arthroscopic suture passing. Sterile, disposable, stainless steel needles are 20-gauge at the tip and taper to 16-gauge at the handle to provide the added stiffness needed for percutaneous placement for arthroscopic suture passing in upper and lower extremities. The ergonomic plastic handle facilitates accurate placement and control. Advancement of a pre-loaded fine nitinol wire loop shuttles any suture through tissue. A selection of straight and curved shafts provides accessibility to dif­ficult to reach structures. &nbsp;
Micro SutureLassos also ac­commodate 2-0 and #2 FiberSticks for single step passing of FiberWire suture and are especially suited for outside/in meniscal repair, rotator cuff repair and TFCC repair of the wrist.

Micro SutureLasso™

The Arthrex Brostrom Repair System allows surgeons to perform a modified Broström-Gould procedure through a single, 1.5 cm incision. The convenient all-in-one system includes 2 BioComposite™ SutureTak® anchors, all the necessary drill guides, the Micro SutureLasso retrievers to help facilitate percutaneous shuttling of the #1 FiberWire® suture through the ATFL and extensor retinaculum.
During simultaneous arthroscopy, surgeons have the option to perform the all-arthroscopic ArthroBrostrom® repair technique using the same instruments from the Arthrex Brostrom Repair System. These techniques offer surgeons 2 truly minimally invasive options to treat patients presenting with lateral ankle instability.

Arthrex® Brostrom Repair

The arthroscopic Brostrom technique can be performed utilizing the Arthrex® Brostrom Repair Implant system. Utilizing BioComposite™ SutureTak® anchors and curved Micro SutureLasso™, surgeons are able to perform an all-arthroscopic lateral ankle ligament repair while simultaneously addressing any intra-articular pathology that may be present from the previous injury.

ArthroBrostrom®

Using standard arthroscopic portals, the Micro SutureLasso™ are directed through the TFCC tissue adjacent to the tear. The Micro SutureLasso can be inserted using either the outside-in or inside-out techniques. A specialized 2-0 FiberStick allows direct passage of 2-0 FiberWire suture through the Micro SutureLasso and TFCC tissue. Using either technique a mattress stitch is created over the tear. The sutures are then tied to the capsule to complete the repair.

For meniscal allograft transplant, the double bone plug technique involves medially implanting the meniscal allograft with rigid fixation at both horn attachments. The technique can also correct meniscal deficiency using bone plugs and minimally invasive suspensory fixation and allows the meniscal allograft to maintain functional hoop stress.

Double Bone Plug

The Double Bone Plug technique for meniscal tear/deficiency utilizes bone plugs that can either be prepared freehand or by using a coring reamer. The freehand technique is performed using rongeurs and a high speed burr. The double bone plug technique for meniscal allograft reconstruction provides a method for implanting the meniscal allograft medially with rigid fixation at both horn attachments. It has been demonstrated that bony fixation and bony healing at the attachment site allows for the maintenance of functional hoop stress by the meniscal allograft.
