Part of the FiberWire® suture family, FiberLoop® is FiberWire suture where both ends of the suture are attached to a single needle, creating a suture loop. FiberLoop is the perfect suture option for multi-strand tendon repairs. These small diameter looped FiberWire products allow for strong multi-strand flexor and extensor tendon repairs while reducing tendon damage from multiple needle passes. FiberLoop is available with multiple needle options to prevent cutting suture while stitching.

FiberLoop® and TigerLoop® Sutures

FiberLoop suture, part of the FiberWire® suture family, has both of its ends attached to a single needle, creating a suture loop. FiberLoop suture is the perfect option for multi-strand tendon repairs. These small-diameter looped FiberWire products allow for strong multi-strand flexor and extensor tendon repairs while reducing tendon damage from multiple needle passes. FiberLoop suture is available with multiple needle options to prevent cutting suture while stitching. The #2 FiberLoop suture is a continuous loop of #2 FiberWire suture on a thin, straight Nitinol needle. The straight needle is easy to handle and moves freely on the suture to recenter itself after passing through tissue and to facilitate even tensioning. #2 TigerLoop™ suture has distinctive stripes to help differentiate graft strands during tensioning.

FiberWire® is available in size 2 with multiple needle options.

Once the minimally invasive or open incision is made and proper soft-tissue windows are established, use the fibular, tibial, and/or femoral collateral marking hooks for anatomic precision and osseous measurements when drilling the appropriate reconstruction tunnels. Using the femoral collateral marking hook with the parallel drill guide will increase anatomic femoral drilling efficiency by significantly reducing convergent tunnels and allowing for accurate guide pin placement at multiple incremental distances. During tunnel creation, use the spooned ends of the collateral ligament retractor to retract the neurovascular structures and the collateral ligament rasp to chamfer the aperture of the completed reconstruction tunnels. Then use FiberWire® or FiberStick™ suture to pass the prepared grafts and tension to the desired position prior to graft fixation.

The Bio-Tenodesis™ Screw System, which includes the <a href="https://www.arthrex.com/shoulder/tenodesis-screws" title="Tenodesis Screw™">Tenodesis Screw™</a>, was designed specifically for the reattachment of ligament and tendon soft tissue to bone. The Bio-Tenodesis Driver facilitates accurate graft tensioning into a bony socket in a simple “push-in” method. The interference fit provided by the Tenodesis Screw and FiberWire virtually eliminates graft separation from the bone. The strength of the repair achieved with the system allows patients to generally begin postoperative rehab earlier than previously permitted. The Bio-Tenodesis Screws are composed of PLLA and are available in numerous sizes to fit all applications. There are also titanium and PEEK Tenodesis Screws available for use with the system, if desired. The system is ideal for the reattachment of soft tissue to bone in upper extremity procedures including rotator cuff repairs, proximal/distal biceps tenodesis andacromioclavicular joint reconstruction.

Many products are available to assist the surgeon with bone-tendon-bone (BTB) graft harvesting and preparation. 

 
 
The parallel graft knife—with parallel blades to create a precise cut in a single pass—is designed for harvesting the patellar or quadriceps tendon during an ACL or a PCL reconstruction. 
 

 
Graft harvesting cutting guides and saw blades can harvest an ideal trapezoidal-shaped bone plug with predrilled suture holes from both the patella and the tibia; the cutting guides provide consistent, reproducible 
results during tendon harvest. 
 

 
Additional instrumentation for BTB graft preparation, including a graft harvesting osteotome and ACL graft shaper, are also available.&nbsp;

BTB Graft Harvest and Preparation Instruments

Multiple collateral ligament reconstruction techniques are available and range from aperture fixation using the BioComposite SwiveLock® tenodesis system with biocomposite interference screws to suspensory fixation using TightRope® technology. 
 
For primary or hybrid collateral ligament repair techniques, choose from a comprehensive line of anchor fixation devices, including FiberTak® soft anchors or biocomposite suture anchors, which come preloaded with needles.

Collateral Ligament Reconstruction Set

Achieve a simple, reproducible repair of the distal biceps with the BicepsButton implant systems and associated tension-slide technique. Using a titanium BicepsButton implant, this technique reliably seats the tendon against the far cortex of the bone socket to maximize the surface area for tendon-to-bone healing. A tenodesis screw adds to the construct strength and helps place the tendon at the more anatomic crescent-shaped insertion. FiberLoop® suture, which is included, also reduces time spent whipstitching the tendon while the button inserter allows for a simplified, less traumatic single-incision repair.

BicepsButton™ Implant Systems

FHL tendon transfer is used for reinforcement of an Achilles repair. Arthrex has developed the Tenodesis Tension-Slide Technique for FHL tendon transfer. The flexor hallucis longus tendon is traced to the calcaneus and harvested. The Tenodesis Graft Sizing Kit is used to determine the tendon diameter and which size implant system to open. The FHL tendon is then transferred to the posterior calcaneus and stabilized with the DX button on the plantar cortex, as well as a Tenodesis screw for aperture fixation.

FHL Tendon Transfer

Arthrex offers the most advanced techniques and instruments for hamstring harvesting and preparation. Harvesting can be accomplished safely and reproducibly both from the traditional anterior approach or through a posterior approach using the minimally invasive hamstring harvest technique and specialized tendon strippers. 
 

The Arthrex Graft Prep Station offers a variety of interchangeable clamps and posts to facilitate graft tensioning and suturing. 
 

Graft preparation has been revolutionized with the FiberLoop® line of suture and SpeedWhip™ technique. The continuous loops of #2 or #0 suture and free-floating straight or curved needles allow reproducible graft suturing in half the time of traditional straight sutures.

Hamstring Graft Harvest and Preparation Instruments

The Interference Screw Technique reconstructs the UCL by using a looped graft anchored into single bone tunnels in the humerus and ulna. The graft is then secured with Arthrex Interference Screws on both sides. FiberWire can also be used for suturing the graft to itself for increased tension and/or original ligament repair for added stabilization.

The SpeedWhip rip-stop technique eliminates the weak link in graft preparation by reinforcing the suture/tissue interface with a FiberTag® scaffold. A FiberTag suture is incorporated into the end of the FiberLoop construct to allow each needle pass to include both the graft and the FiberTag scaffold. This construct has been shown to increase the strength over standard stitching alone.1

Looped Suture With Needles

Tenodesis Screws™&nbsp;may be used in conjunction with #2 or 2-0 FiberWire®&nbsp;to facilitate intraoperative tissue tensioning and fixation in a predrilled socket. The predrilled socket minimizes incision length, dissection, and overall morbidity. BioComposite, PLLA, vented PEEK or titanium Tenodesis Screw insertion provides superior and immediate fixation for foot and ankle indications such as Achilles repair, FDL, FHL tendon transfers and lateral ligament stabilization. The system can also be used for applications in the hand and elbow (UCL, LRTI, distal biceps), shoulder (rotator cuff repair, proximal biceps), as well as collateral ligament repair, reconstruction and secondary graft or suture fixation for ACL, PCL reconstruction. This construct allows for direct tendon-to-bone healing, without hardware prominence. &nbsp;
The Tenodesis Screws are part of the Bio-Tenodesis Screw System which eliminates transosseous tunnels in tendon repairs and ligament reconstructions by simplifying tissue fixation in a bone socket while maintaining tension of the tissue throughout fixation.&nbsp; A specially designed, retracting screwdriver with extended tip tensions the graft near the base of the socket while a unique outer sleeve delivers the Tenodesis Screw as the extended driver retracts. This technique is ideally applicable for biceps tendon and collateral ligament repairs. The graft passing sutures through the screw cannulation can be used to provide backup fixation by incorporating anti-slippage mulberry knots at the distal end of the screw. The suture ends can be tied over the screw at the completion of the procedure as additional backup fixation, if desired.

As part of the distal biceps repair procedure, the tension-slide technique with the BicepsButton™ implant provides a simple, reproducible, and biomechanically stable repair. This tensioning technique reliably draws the tendon against the distal cortex of the bone socket. Adding a tenodesis screw improves the biomechanical strength and allows the tendon to be placed in a more anatomic position.
The all-suture FiberTak® button can be used in lieu of traditional metal buttons for both bicortical and unicortical tension-slide tenodesis techniques.
