Sutura SutureTag

FiberTak® Button With SutureTag Looping Stitch

Hi, my name is Aurélien Bio-Post and I'll be presenting the SutureTak Looping Stitch. The SutureTak Looping stitch creates a running rip stop to reinforce the tendon with minimal passes through tissue. This novel stitch is lower profile, faster to complete, and stronger than a typical Krakow repair stitch. The SutureTak splits into two number two sutures that can be easily incorporated into the FiberTak button repair technique. I will be demonstrating the SutureTak looping stitch technique to perform a sub pectoral biceps tenodesis with the FiberTak button. Here we have an open sub pectoral exposure, and I've removed some of the pectoralis so that we can demonstrate where the tenodesis will take place. We have the tendon already prepared and we can start the SutureTak looping stitch technique. You begin by piercing the tendon and pull it through until you've reached the junction of the flat tape and the double number two suture limbs. The tape portion lays flat along the tendon end and it is secured with a clamp. Now that we have both ends of the tag portion of the suture secured to the tendon, we can start the looping stitch technique. First, you want to wrap the suture completely around the tendon and then pass the needle through the suture tag and tendon, taking care to make the pass behind the wrapped suture to create a rip stop. You want to make sure that you tension each of the suture limbs individually to remove any slack from the construct. Notice that with this technique, you don't have to separate the sutures and come back around the clamp, as with other techniques. In fact, this technique can be done with any repair performed in situ or any graft preparation. With the graft still on the prep station and clamped for the second loop. We will do the same thing. We'll wrap the suture completely around the tendon, and then pierce back through the tag and tendon behind the suture. Limbs. Tensioning the limbs individually. The third and final pass is completed the same way, coming around the tendon and then passing it back through like we did before creating a running rip. Stop. Now you can cut off any excess tendon and suture to prepare for the incorporation into the FiberTak button. So here you see the bicipital groove and we can drill into the desired location for our FiberTak button using the drill guide and the 2.6mm drill. Next, while holding the guide in place, insert the FiberTak button through the guide and mallet until the inserter is flush with the guide. Now we remove the orange tab and remove the inserter and guide. We set the anchor by pulling back on all the sutures to deploy the button. Now we separate the FiberLink sutures. We have the blue and white suture and the black one on each side. After cutting off the needle from the SutureTak suture, we have two separate limbs. So now we're ready to shuttle the first suture limb using one of the FiberLink sutures. A few short, quick tugs will help to pass the suture through the button, but be sure to hold tension on the opposite sutures to ensure the button stays seated. Now we can load up the other suture lemon to the remaining FiberLink, and similarly shuttle it through the button. We use the tension slide technique to pull the biceps securely into place using a free needle. Pass one suture limb through the tendon and tie to secure the repair. Now we can cut the excess suture. Here we demonstrate the suture tag looping stitch used to perform a sub pectoral bicep tenodesis using the FiberTak button.

Subpectoral Biceps Tenodesis Using the FiberTak® Button With SutureTag Looping Stitch

The SutureTag looping stitch reinforces the tendon at the repair site and increases the contact area of the suture-to-tendon interface while providing a running ripstop configuration. This animation shows the SutureTag looping stitch being used with a FiberTak® button for subpectoral biceps tenodesis.

Subpectoral Biceps Tenodesis With FiberTak® Button and SutureTag Looping Stitch

Make a 2 to 3 centimeter incision in the axilla at the inferior border of the pectoralis major, bluntly dissect to identify the pectoralis major and the long head of the biceps tendon. Retrieve the previously synonymized biceps tendon through the incision. Prepare the tendon at the desired length and pierce the tendon at the transition point of the suture tag to secure in place while holding the other end of the clamp. Next, wrap the suture completely around the tendon and then pass the needle through the suture tag and tendon at the starting point. Take care to pass the needle behind the suture, limbs on the underside to create a rip. Stop. Be sure to tension each limb independently to ensure the stitch is secure. For each subsequent throw, the suture is wrapped around the tendon and then secured in place by penetrating through the suture tag and tendon. The needle and suture limbs should be passed behind the wrapped suture limbs to create a running rip stop. A third row around the tendon and through the SutureTak and tendon is recommended. Cut at the transition point to remove the needle. Taking care to leave enough length for shuttling the suture tag. Looping stitch is now complete and ready to be incorporated into the FiberTak button. Place the FiberTak button drill guide at the desired location on the bony surface of the proximal humerus, and drill a cortical hole with a 2.6mm spade tip drill. Keep the drill guide in place and insert the FiberTak button through the guide. Lightly tap on the inserter handle until the inserter is flush with the drill guide. Remove the orange suture release tab to remove the sutures from the inserter handle. Remove the implant inserter and drill guide. Lightly pull evenly on the FiberLink to assure the button is deployed. Separate the FiberLink blue and white black on each side and pull each link to ensure it slides. Shuttle one of the SutureTak limbs through the button using one of the FiberLink. Use slight tugs once the suture meets the button sheath, hold slight tension on the opposite suture links while shuttling to ensure the anchor remains in place. Pull lightly on the remaining shuttle link to verify that the button is set. Shuttle the second SutureTak limbs using the remaining link in the same manner. Use the tension slide technique to reduce the tension onto the bone. Pass one limb back through the tendon and tie a knot to complete the repair. The FiberTak button can also be implanted by Cortically for an inlay repair in conjunction with a tenodesis screw.
