RCL and FDP Repair Using the Nano FiberTak® Soft Anchor

Speaker  Today we're using the Arthrex all suture anchor. It's the 1.0 Nano FiberTak anchor. It's an all suture anchor with a 0.6mm SutureTape for the repair stitch, which has tapered needles on it. And it's an equivalent strength of about 3.0 FiberWire. You'll see there's a blue stitch on there. It's a number two FiberWire retention stitch that needs to be removed. And then the drill next to it is a drill bit. That is a small step drill. The full length of the drill bit is ten millimetres, but the first eight millimetres of that is one millimeter in diameter. The final two millimetres is 1.1mm in diameter, and this drill can be placed on a pin collet when utilizing it. Today we're working with a specimen here that's already been dissected out. This would potentially be for an FDP repair in a finger or a volar capsulorhexis, perhaps in the thumb MCP joint for volar plate advancement on a hyperextension instability case. You can see here we've taken out the radial collateral ligament. So simulating a rupture you can see here the ligament is or the finger joint is very unstable. And we're going to go ahead and repair this back to its native position right here. I'm going to go ahead and mark this just so it's a little bit visually easier to see, but the insertion site is right around that area, right around that spot. Speaker  And so we'll go ahead and utilize this drill again. This drill is a 1.0mm drill up to the first eight millimetres. And then the final two millimetres here is a 1.1mm drill as well. It's a step drill. You can go ahead and put this drill on a regular pin collet, and you want to make sure you drill the entire ten millimeter length. Okay. And you can obviously see we're using fluoro here. Of course, I would always use fluoroscopy anytime I'm doing this type of procedure. And you can see the drill is nicely bottomed out. We have plenty of room here in this specimen. We've got this drilled completely to the ten millimetres. I want to just show this anchor here relative to my hand size. You can see it's extremely small. And you can also see those blue sutures on the top there. It's the number two FiberWire. That's the retention suture that you're going to ultimately pull. The other thing I want to show here is on the backside of this anchor is the plastic retaining sleeve is right here at the end of it. When you insert this, it should bottom out against this metal positive stop here and you'll hear an audible click. When that happens, to know that you've actually seated your anchor far enough, you can either do this by hand or if you have hard bone like we do in this situation here, we might have to actually do some gentle taps with a mallet. Speaker  You can see this plastic retaining sleeve is all the way back against this metal stop, signifying that we have seated the anchor far enough to deploy it. We're seated here with the anchor. There's a black retention ring in the back of this anchor, like many of the Arthrex anchors have, and then we will pull the needles out, which are protected. This is that number two, FiberWire retention suture. And then again, this number two, FiberWire was just solely there to be a retention suture. So we can pull that out. And now we have our repair stitch. Okay. And you'll see once this anchor has been seated in here and we've taken all the sutures off, there's a little loop of suture here that's very normal to be outside of the bone prior to actually setting the anchor. Then you're going to take both ends of the suture here and sequentially go back and forth and it'll suck that loop back down in there. And you can see it just disappeared into the bone. And now we're nice and free. You can also see that the suture, once it's been set is a sliding suture here. And so we're going to take one again. You can use whatever repair suture or technique that you prefer. I'm just going to for simplicity show you this very simple repair stitch. Speaker  One of the things I like about this anchor is that it's a SutureTape. It's a 0.6 SutureTape, which is essentially the same strength as a 3.0 FiberWire. The advantage, of course, is that SutureTape is a flat suture, and it lays nice and flat against the side here. So you can see when we pull that back, it's right against that footprint. And then you can tie it to its partner. Next. The other suture here, the extensor mechanism can be closed to whatever technique you normally utilize. And obviously post-operative protocol per your own preference. The other nice thing about this anchor is because of their small size, you certainly could put two close together or two side by side with each other. If you wanted a second anchor for additional stability. Okay, you can see here that we have a specimen that's exposed for an FDP repair, just so you can kind of see how we push this anchor in. Again, if the anchor doesn't go in easily, you can certainly tap it in. Or if it goes in relatively simply, you can just try to push it in and you can hear that snap. You can also see at the back end of that where the plastic has met the metal piece, and that's that sound that you just heard signifying that we were good to go.
