High Monocyte Technique With the ACP Max™ PRP System

Speaker  Hello everyone. My name is Alberto Pinero from Sacramento, California. Today we're going to be going over the ACP Max monocyte rich protocol with a 90 ML blood draw. The idea here is to be able to reduce your neutrophil count while still increasing your monocytes. We want to take advantage of the M1 to M2 polarization and hopefully create an anti-inflammatory effect. This technique also allows you to keep, on average, a 7 billion platelet dose on your final PRP product. The first step is to remove the ACP canister and remove the air. Second, you want to add the red cap. Then you want to remove this guide to accommodate the 60 ML blood syringe. Next, we're going to grab the 60 ML blood syringe and engage it into our canister. Make sure it's nice and locked. And we're slowly going to introduce the blood into the canister. Next, we're going to place the syringe guide. And then advance the 30 cc syringe. Now we're going to go nice and slow here not to create any turbulence. Also noting that since we're doing the 90 ML draw, we're going to have an excess of Pep. So we don't need to fill our full 30 cc syringe. We can stop a little bit over halfway and then change to the next syringe to not disturb the Buffy coat. Now we've added our second 30 cc syringe. I want to note that the Buffy coat is right below the 40 tick mark for the monocyte rich protocol. This is. The second key differentiating step is that we are going to stop three ticks above the red blood cell layer. So we're going to gently start to pull. We're going to continue to pull our p p nice and slow, ensuring that we stop three ticks above the red blood cell layer. Now we can remove the 30 cc syringe and get our ACP syringe. We want to make sure that we prime the air out of it. Speaker  And also as we advance it, we want to make sure that we keep some pressure in the smaller syringe so that the blood doesn't come up. Now that we are engaged, we're going to go ahead and pull and fill our double ACP syringe to 15 cc's. Once that's completed, we're going to go ahead and put the red cap. Now we are ready for our second spin at 1500 rpm for three minutes. Ensure to place your blue spacers into the buckets as well as your counterbalance. This is another critical step to make sure you homogenize the product. Go ahead and invert the ACP syringe 15 to 30 times. Now we can start our second spin at 1500 RPMs for three minutes. The second spin has been completed. We can now remove the double ACP syringe and get ready for final processing. Now we are ready to extract our PRP. Noting that for the monocyte rich protocol, we want to keep going down until we see a flash of red. Now that we've seen the flash of red, we want to pick a landmark within the smaller syringe and aspirate an additional CC. Now that the aspiration has been completed, we can remove the smaller syringe. Place a cap on it. We're going to invert it multiple times to homogenize our final PRP product. And now we have our final PRP product which is monocyte rich. This concludes the centrifugation protocol.
