Speaker  So how do you plan the perfect incision, and what is the ideal position and trajectory of the pars screw? So ideally you want the screw to have a medial to lateral trajectory on the AP plane with bisection of the pedicle on the lateral view. You want to ensure the trajectory is in line with the pars with the endpoint at the top of the pedicle. Now, whether using navigation or biplanar fluoroscopy, it's critical to ensure the incision is sufficiently distal and adjacent to the midline to allow for drilling for optimal screw placement. Be sure to dissect past the fascia to ensure your drill guide is not being held up by soft tissue.
