Distal Humerus Fracture Fixation With the Arthrex Distal Humerus Posterolateral Plate

Hey, this is Kevin Farmer from University of Florida. Sports Medicine and shoulder and Elbow demonstrating the new poster level plate from the distal humeral system. Some benefits of this plate. I'd like to highlight one being the low profile nature of it. Number two is having the multiple screws in the capitellum. So four locking screws in this particular plate sort of maximizing that fixation in the capitellum. You can see from these different holes there also what we call crew lock holes which are locking compression type screws. Both two seven and three five fit in all the holes. So you have the opportunity to choose one screw based on your preference. I also want to point out this lateral tab here, which allows you the opportunity to place a screw across the entire spool, sort of maximizing that interdigitation and maximizing the principles of of fixation into the distal humerus. That tab can be as malleable and can be positioned exactly where you want it. Or if you don't need it, it can be simply removed by cutting it out. All right. We've transitioned from the back table now to our surgical field. We've done a standard olecranon osteotomy approach to distal humerus. Here you see the ulnar nerve we've tagged and we've moved that out of our way. We have the posterolateral plate provisionally fixed in place with these BB tacks. We use this long end back elevator to help us expose the distal humerus that's on the set. It's a nice addition to have the plate you can see did not need to bend or contour. It fit perfectly. Even the the lateral tab here fits very nicely in place. We're going to place our initial fixation into the humeral shaft through the oblong hole using A35 cortical screw. Take our depth gauge. All right. We're placing our three five cortical screw here. This is the freehand point and shoot guide for either freehand or locking screws. I want to point out the single laser mark is for use for measuring up to the 50 millimeter screw length. You see that as that's going up now to the 50. And as you pass the 50, then this double mark here at the other end will be your measurements for for screw lengths greater than 50. As you see as that goes forward now and you measure that on the, on the other side of the the guide, we're using the distal point and shoot guide here for our one of our distal capital screws up. Get that going. So hitting bone again following O'Driscoll principles getting as long of a screw as possible. That is measuring a 34. And then confirming with the depth gauge again also measuring a 34. All right. Now we're going to place a crew lock screw in our tab here to get that fixation across the entirety of the spool. We have our locking guide in place. We have a long depth gauge here for screws that measure anything over 60. And so we'll just confirm in this case we're measuring 58 for that screw across the spool. All right I do want to again point out we're placing the crew lock screw. So it is a locking compression screw. So you get compression as it pulls that tab down and then the screw locks into it once you've achieved that compression. So nice ability to have kind of the combination of the two there. All right. And the AP image we're checking the screws and screw length provisionally all look good. And this lateral image we're again looking at making sure there's no penetration in the articular Surface checking, screw length, plate positioning and again all looks good in the lateral image as well. Here we see final AP lateral and oblique images showing appropriate placement of the plate. Good compression of the plate to bone and appropriate length of all screws. You can see from this AP image. This lateral tab gives you access to a screw across the entire spool. In this case, we use the variable angle aspect of it to achieve the appropriate direction and screw length. The most distal locking screws of this plate are designed that if you use the locking guide to avoid the locking screw from the tab. If you decide to use variable angle screws in the most distal aspect of the plate, then you should use a variable angle point and shoot guide for your tab screw just to avoid screw traffic.
