James L. Chen, MD, (San Francisco, CA) demonstrates the use of AlloSync™ Pure demineralized bone matrix mixed with concentrated bone marrow to fill an ACL socket in a revision surgery. The mixed putty is delivered into the socket using the BioXpress™ graft delivery device.

Revision ACL Femoral Tunnel Fill With AlloSync™ Pure Demineralized Bone Matrix

This is AlloSyncPure. And what this is, is a demineralized bone matrix that comes in a five cc jar. And it does not have an active carrier. One nice thing about it is you can add a biologic fluid to it to create a sort of bio putty. And so you have to add 40% fluid. And this is a five cc jar. We're going to put two cc's of bone marrow concentrate in. And then I'm going to mix it together to form the AlloSyncPure. So there's two CCS that go in. And then I am going to use this spatula to mix it. Now you can see these particles are very thin and they've been processed in a specific manner so that there's more surface area. And so that once the biologic fluid is added the particles will stick together and nest together. And I'm able to create a a biologic putty using the bone marrow concentrate. With the AlloSyncPure. You have to mix it for two minutes in the jar, and then an additional minute in your hand molding it. So after you've mixed this for two minutes in the jar, then you're going to transfer it to your hand and you can hand shape it for an additional minute to give it some more shape here. And you can see it's a real putty and it has shape now and is more firm. And after you've shaped it for an additional minute, then you can transfer it to the Arthroscopic transfer device, which is the BioXpress. So this is the BioXpress arthroscopic delivery device. And what's innovative about this is it has two plungers. You can see here you can put material in the large syringe and then you can plunge it down. And then it will go into the arthroscopic cannula once the biological material is in the arthroscopic cannula. You can use the internal plunger to advance and then express the material into the defect or desired area. And this is a revision knee. And we are going to use the AlloSyncPure which is the DBM with mixed bone marrow concentrate. And I'm going to backfill this socket using the BioXpress arthroscopic delivery cannula. I've gone ahead and introduced the BioXpress delivery device. And I'm going to put in the AlloSyncPure. And one nice thing about the AlloSyncPure is that it's 100% bone, so it will not migrate or leak out. And now I'm pushing the internal plunger and you can see the AlloSyncPure going into the socket. So I'm looking through the anteromedial portal and you can see that we've backfilled nicely this femoral socket with the AlloSyncPure. You can see that the material is staying in place and is not going anywhere. Typically in a revision setting, it will take 2 or 3 months for bone graft to heal. We do have data showing that at 28 days there are all five bone forming elements present.
