Case Presentation: Arthrex VA Proximal Tibial Plating System

Speaker Hello. My name is Ben Maxson and I am a fellowship trained orthopedic trauma surgeon, and I practice in Tampa, Florida. Today we're going to review a case presentation utilizing the Arthrex variable angle proximal tibial plating system for tibial plateau fractures. The Arthrex proximal tibial plates are comprehensive. There are lateral, medial and posterior medial plates. And of the lateral plates there's two different designs. There's a standard proximal contour and there's a high proximal contour which allows for additional screws proximally and fit for people with bigger bone. When we take the Arthrex proximal tibial plates and we combine them with other Arthrex products, such as the mini fragment system, we can treat essentially all different types of proximal tibia fractures, even when they extend to the metaphysis and diaphysis. And we can do that without compromising the quality of our reduction and fixation. Biologic augmentation is important to talk about, and Arthrex does offer different options for that. Things such as Quickset, which would provide more of a structural support. Bonesync, which is somewhat structural but also allows for incorporation of biologics to promote osteoinduction and then AlloSync, which is bone chips to allow for void filling. Here's our case that we'll talk about in a little bit more detail. This is a 72 year old female who sustained a ground level fall. Speaker Her past medical history is pertinent for hypertension, diabetes and hyperlipidemia. She sustained a lateral split depression, injury, and on this injury, there was also extension through the tibial spine into the medial condyle. We highlight this one because this is a very common injury pattern. You can see on the plain film imaging that there is quite severe depression of the lateral joint surface and comminution. And as we move on to our CT scan, this is the axial image. We can see the comminution of the joint surface and highlighting the amount of depression. The coronal CT scan, which we see here, also shows us the amount of depression of the articular surface, as well as the amount of comminution for our surgical plan. We're going to plan for a lateral approach to the proximal tibia. We are going to expose the lateral aspect of the lateral condyle, both anteriorly and posteriorly. We will perform a lateral meniscal arthrotomy and we will look for a possible meniscal tear, which in this scenario is likely to have occurred with a peripheral avulsion tear of the lateral meniscus. Here we have our final images of our tibial plateau. After fixation, we use the standard lateral plate, which was appropriate for the position of the subchondral raft screws, whereas the high lateral plate would provide an additional row of screws if needed. Speaker More proximally Post-operatively. What we'll plan for is non-weight bearing but immediate range of motion. I don't generally immobilize patients, and I don't generally place them in hinged braces unless there are other concerns, such as Ligamentous injury. This patient is going to maintain non-weight bearing for about 10 to 12 weeks, and that depends on what we found intraoperatively with regard to their bone quality as well as what we see radiographically with their progression of healing. Here's a couple more examples from my practice here is that Bicondylar plateau that we referenced earlier with the extension into the Diaphysis. In this case, we use the mini fragment set to restore the alignment at the Diaphysis and restore the shaft to the articular block, and then use the lateral plate to support the Bicondylar plateau fracture, and also used a subchondral graft screw on the medial side as well. Here's an example of a medial fracture. And in this case we use that posterior medial plate to buttress the medial articular surface and prevent it from sliding posteriorly. And we also use RAF screws to support the subchondral surface of of the plateau. Thank you. And that concludes this presentation on the Arthrex variable angle proximal tibia plating set.
