Interlaminar Endoscopic Lateral Recess Decompression

Speaker  The Arthrex Interlaminar approach for a lateral recess. Decompression is an ultra minimally invasive surgical technique ideal for patients diagnosed with lateral recess stenosis. In this procedure, an endoscope is used to assist with access and visualization. Place the patient in the prone position and use a C-arm to verify the correct operative level using both AP and lateral images. This should be confirmed based on the preoperative MRI. Use a switching stick to identify the entry point target between 9 and 12:00 in the Interlaminar window. For a left sided decompression or between 12 and 3:00 for a right sided decompression, Mark the incision site. Insert an introducer needle. Remove the stylet and pass a guidewire through the cannula. Make an incision approximately one centimetre in length through both the skin and fascia. Remove the cannula and insert a switching stick or serial dilators over the guidewire. Insert the cannula over the switching stick, the surgeon may choose to connect a cannula holder to help maintain the trajectory. This should be attached prior to removing the switching stick. A depth stop may also be attached to the endoscope to maintain the appropriate depth within the cannula. Once the endoscope is inserted, introduce a series of grasping instruments, whisker shaver blades and an electrosurgery probe through the working channel. Speaker  Arthrex offers unique whisker shaver blades to help facilitate efficient controlled soft tissue removal. Remove excess tissue and coagulate blood vessels to identify anatomical landmarks and expose the ligamentum flavum, making sure to expose the junction between the lamina and the ligamentum flavum Arthrex offers a variety of shaver burs, including match head, coarse diamond and fluted designs for efficient bone resection. Rest the bur on the ligamentum flavum undercut the ventral cranial lamina at the ligamentum flavum attachment until it detaches from its cranial extent. Follow the lateral aspect of the cranial lamina to the medial facet and caudal lamina. Rest the burr on the ligamentum flavum and undercut the ventral caudal lamina until the ligamentum flavum detaches from its caudal extent. Extend the drilling laterally to the medial border of the superior articular process. Resect the ligamentum flavum using kerrison rongeur or grasping instruments to fully expose and decompress the traversing nerve root. Use a ball tip, probe and assess for adequate decompression of the lateral recess and mobilization of the traversing nerve. Remove the endoscope and cannula and close the incision. Apply JumpStart antimicrobial wound dressing over the incision site.
