Delaminated Rotator Cuff Repair Surgical Technique Animation

When treating rotator cuff tears, it's important to first assess the tear pattern of the rotator cuff to understand how the tendon will reduce to the footprint. In some cases, you'll find a delaminated cuff tear in which the layers of the rotator cuff will reduce to the footprint at different points. In cases like this, a dual layer FiberTak SpeedBridge construct is used to address the layers of the tear. With the first 2.6 FiberTak RC set in bone, retrieve the repair suture from the anterior anchor through the lateral portal. Load the repair suture into the Scorpion suture passer and pass the repair suture through the inferior aspect of the delamination. Retrieve the loop end of the shuttle suture from the lateral portal. Load the repair suture into the loop, folding at the purple mark. Convert the knotless mechanism by pulling the SutureTape end of the shuttle suture from the percutaneous portal. Next, pull the repair suture to tension the inferior aspect of the delamination, but do not fully reduce the loop tension. Each repair suture individually to fully reduce the inferior aspect of the delamination. Repeat these steps for the posterior and middle anchor. Once proper tension is applied, cut the tails of the repair sutures. Retrieve the anterior FiberTape. Load the joined tails into the Scorpion suture. Passer and pass the FiberTape through the cuff. Repeat this step for the other two FiberTape sutures. Once all the tapes are passed, cut the joined tail of the FiberTape sutures. Retrieve one limb of FiberTape from each anchor and use the Mega Loader to load them into the Self-punching. Knotless. Swivelock. Anchor with the eyelet above bone. Remove slack from each limb of FiberTape with all slack removed. Press the Self-punching SwiveLock anchor to bone and maintain forward pressure. Use a mallet to tap lightly to advance the eyelet through the cortex. Mallet until the anchor body is touching bone. Once the anchor is touching bone, advance the SwiveLock anchor in place. The lateral knotless mechanism can be used for cable reinforcement. This completes the FiberTak SpeedBridge construct, allowing for differentiated tension of the layers of the delaminated tear.

CuffMend™ Rotator Cuff Repair Augmentation System

The CuffMend system provides a fast and efficient technique to augment partial and full thickness rotator cuff tears using ArthroFLEX dermal allograft. Prepare the graft by adding a FiberLink SutureTape in a cinch configuration to the lateral corner of the graft. Repeat for the other side of the lateral graft. Tigerlink suture. Pass the suture through the medial corner of the graft. Repeat for the second medial suture. Placement. Load the ArthroFLEX graft construct onto the graft spreader by first loading the tails of the medial number zero FiberWire suture through each corresponding arm of the graft spreader. Dock the medial number zero FiberWire sutures into the inner cleats located on the handle of the graft spreader in a crisscross pattern. Dock the lateral TigerLink, SutureTape and FiberLink SutureTape sutures straight back into the outer cleats of the graft spreader. After loading the ArthroFLEX graft onto the graft spreader, retract the arm by sliding the button in the direction away from the graft. Insert the graft and graft spreader through the lateral passport cannula. Once the tips of the graft spreader arms enter the subacromial space with sufficient clearance. Deploy the graft spreader by sliding the button forward to fully deploy the graft through the percutaneous portal. Penetrate the tissue with the FiberStitch device until the depth stop is flush with the graft, maintaining slight downward pressure. Roll the deployment wheel backwards until the hard stop, and an audible click is heard to deliver the first implant. Roll the deployment wheel forward to the hard stop to load the second implant remaining within the subacromial space. Advance the needle tip through the tissue in the desired location. Roll the deployment wheel backwards until the hard stop and an audible click is heard. Roll the wheel forward to complete delivery of the second implant. Upon removing the device from the joint, a suture loop and a loose end of suture will be outside of the shoulder. Pinch all the sutures and gently pulled to set the implants. Identify the suture loop. Use a probe or similar device to pull the loop. Suture to compress the suture anchors. Tighten the construct by pulling the single suture to reduce. Introduce a suture cutter to cut the suture on top of the graft. Repeat this process with a second FiberStitch device for the interior. Medial fixation of the graft. Un cleat and cut one end of one of the medial graft retention sutures before pulling on the opposite limb for removal. Repeat on opposite corner. Introduce the PushLock anchor to the desired location and insert the PushLock. Taking care to not over tension the graft construct. Repeat for the second 3.5mm. Self-punching PushLock. Suture. Anchor to complete fixation.

Tensionable Knotless Biologic Tuberoplasty With ArthroFLEX® Dermal Allograft

Tensionloc Knotless biologic Tuberoplasty harnesses the power of the ArthroFLEX dermal allograft to provide a biologic cushion between the acromion and the tuberosity that can prevent bone on bone contact due to an irreparable rotator cuff tear. Prepare the greater tuberosity by mildly decorticating the surface using an arthroscopic burr or PoweRasp using an arthroscopic ruler. Measure the anterior to posterior footprint. Measure the medial lateral dimensions of the footprint. Note to undersize the graft by 15 to 20% using a Scorpion suture passer. Prepare the anterolateral corner of the ArthroFlex graft with a 1.3 FiberLink SutureTape in a luggage tag configuration. Repeat for the posterolateral corner. Prepare the graft on the anteromedial corner with a FiberLink in a simple pass, noting the loop to be under the graft. This loop will later be used to shuttle the repair suture from the anteromedial anchor through the graft. Repeat for the posteromedial corner. Through a percutaneous portal, place a 2.6 FiberTak guide onto the anteromedial position of the footprint and insert a self-punching Knotless 2.6 FiberTak soft anchor. Repeat for the posteromedial corner. A passport divider may be used for suture management. Retrieve the number five. Repair suture and round shuttle link from the anteromedial anchor through the lateral portal. Repeat for the posteromedial anchor. Use the FiberLink to shuttle the anteromedial. Repair suture through the anteromedial corner of the graft. Repeat the steps for the posteromedial anchor. Load the number five. Repair suture into the loop of the shuttle stitch and fold it over at the purple mark. Pull the tape end of the shuttle link from the percutaneous portal to convert the knotless mechanism. Repeat the steps for the other medial corner. Remove the passport divider holding counter tension on the lateral luggage sutures. Sequentially pull tension on the anteromedial and posteromedial. Repair sutures to shuttle the graft through the 12mm passport and onto the prepared greater tuberosity. Load the luggage tagged FiberLink SutureTape into the eyelet of the self-punching SwiveLock anchor. Apply tension to the FiberLink SutureTape to achieve the desired fixation and insert the SwiveLock anchor. Repeat for the posterolateral corner of the graft to complete the tension. Double Knotless biologic Tuberoplasty. A partial repair can also be incorporated with a biologic tuberoplasty construct.

The Knotless FiberTak® soft anchor has a tensionable, self-locking mechanism that helps create an adjustable, knotless soft-tissue repair. Two anchors may be interconnected to create an adjustable, knotless, bridging construct.

Knotless FiberTak® Soft Anchor: Interconnected Remplissage Technique

The Knotless FiberTak® biceps implant system offers a knotless, all-suture anchor solution to perform an onlay proximal biceps tenodesis at any location. The implant uniquely integrates two individual tensionable knotless mechanisms within a single FiberTak sheath. The blue FiberWire® repair suture passes around the biceps tendon, while the white suture loop is retrieved through the tendon. The blue repair suture is retrieved through the loop, and both are tensioned to complete the tenodesis.

Knotless FiberTak® Biceps Implant System for Proximal Biceps Tenodesis

In the subacromial space. Insert a cannula in line with the intended anchor insertion site. Place the drill guide on the bony surface of the proximal humerus at the desired tenodesis location. Drill a pilot hole using the 3.0mm. Drill until the positive stop on the drill guide is reached. Keep the drill guide in place and insert the Knotless FiberTak implant through the guide. Lightly tap on the inserter handle until it is flush with the drill guide. Remove the black suture release tab to remove the sutures from the inserter handle. Lightly pull evenly on the suture limbs to set the anchor in the bone. Using a suture retriever, shuttle the solid blue FiberWire. Repair suture under and around the biceps tendon to the opposite side of the white suture loop. Insert the loop and tack SwiftStitch. Suture passer and penetrate the biceps tendon at the desired tenodesis location. Retrieve and shuttle the shuttle the white tensioning suture loop through the tendon. Using a suture retriever. Retrieve the solid blue FiberWire. Repair suture through the white TensionLoc suture loop. Load the blue FiberWire. Repair suture through the shuttle link suture loop and fold the repair suture limb at the purple indicator mark. Pull the tape end of the shuttling suture to shuttle the repair suture through the knotless mechanism and tension the repair suture around the biceps tendon. Tension the white suture limb to lock the tendon to the blue circumferential stitch. Pull each suture limb sequentially to ensure adequate tension and a secure tendon reduction. Cut the suture. Limbs. Excise the biceps tendon 5 to 10mm proximal to the repair, and remove the remaining tendon stump from its attachment to complete the repair.

The QuickPass™ FiberTape® Cerclage System features cannulated, slotted passers that allow simplified passage of FiberTape Cerclage suture around bone for fracture management. The QuickPass tube on the FiberTape Cerclage suture facilitates quick insertion through the cannulated hook, and the suture is removed from the slot to complete a wrap around the bone without the need for a separate shuttle suture.
