BioACL™ Technique Using the GraftNet™ XL Device

I'm Doctor Chad Lavender with Marshall University orthopedic sports medicine, and we're going to highlight a case of the bio ACL featuring the new Graphnet XL. This patient will go over the history and exam. So this patient is an 18 year old female. She's status posed a non-contact injury to the right knee. On examination she had a positive lochman positive pivot shift, full range of motion and an effusion. Her diagnosis after MRI was a right knee ACL rupture. In this case, we used the ACL, which combines biologics and an internal brace in the form of the new tightrope two to do the ACL reconstruction. And we used the quad tendon autograft as our choice of graft. So let's talk about the integration of the Graphnet XL and the ACL technique. Basically we first start with harvesting bone marrow aspiration from the proximal tibia. And that gets spun down with the Arthrex angel system. After that we take the autograft bone that's collected with the Graphnet XL device, and we mix that with the pure into a composite graft. That composite graph then gets injected into the femoral tunnel and the tibial tunnel before passing the quad tendon. Autograft the Graphnet XL steps, we first connect the graphnet XL tubing to to the desired suction, and after that we collect the autograft bone through the internal filter in the graphnet XL. Next, we remove that filter, we squeeze out the remaining fluid, and then we invert the filter and remove the bone onto the back table. Here's a surgical technique video showing how we place the Graphnet XL off the field, and we collect the autograft bone. As you can see here, it's it's it's amazing how much autograft bone that you can filter and get into this Graphnet XL, and it also allows you to do more collecting without changing out your graphnet. So once that is collected, we squeeze out the remaining fluid on the back table, and then we'll invert the Graphnet XL filter, and we remove that with a freer or an elevator or our hands and place that into a collection. As you can see here, here we removed about three CCS of autograft bone, but you can get an abundant amount more for our mixture. We used 2 to 3 CCS of BMC, one cc of autograft bone harvested with the graphnet XL, and five ccs of pure a pearl or a couple pearls of the technique. You want to squeeze the back of the filter to remove any remaining fluid from the bone, and then after that, you want to remove the actual bone onto the back table. And we like to clamp the graphnet XL away from the field, which really gives us freedom to work and do the ACL reconstruction. Thank you so much for your time.

GraftNet™ XL Device for Spine Procedures

I'm Andrew Clavenna from Dallas, Texas, here to demonstrate the Graphnet XL. This is a case standard mis-tlif type case important when collecting bone dust that use different suction lines so that we don't put soft tissue into the bone dust collector. So here we are. Tube retractor facet joint cephalad caudal. And I'm just going to clean off some of the soft tissue from this facet joint as we would standard in any case. Here's where simply having the multi-port allows us to switch over to the suction tubing to collect our bone graft. Obviously, a three millimeter matchstick two fluted device is better, as studies have shown flute size does make a difference. Definitely don't want to use like a diamond bit, bur as our cell viability would be much less benefit of that. Graphnet XL is that the mesh collector allows us to irrigate without worry that we're diluting our bone graft. Clean. Classically, all this bone would be thrown away and wasted and allowing us to harvest this and use it for our fusion has shown that this bone has mesenchymal cells. And osteogenic potential to facilitate our fusion during our tlif. So I've removed most of the inferior articular process of L4, and now going through the superior articular process of L5. One of the advantages of the graphnet XL is that it does have a three foot suction tubing attachment to get your hand away from the collection device, decreasing weight on your hand to minimally change how you normally operate, as well as allowing the scrub tech direct visualization of your collected bone grafts so they can notify you if you're filling up. The device does allow for a large volume of collection. It also has a clip attached so that the scrub tech can clip it to the mayo, or hang it from the mayo so that it's out of the way and you don't have to worry about it falling onto the floor and contaminating the sterility and using the drill to remove this bone. It allows us to get down to that cortical edge and overall minimize the use of the kerosene. Less kerosene. Use less fatigue you have to worry about in your hands. It's important after you're done collecting your bone dust, your suction tubing attachment, especially if you're using the three foot adapter, has extra dust in it, and you'll want to collect that by simply irrigating through the suction device. As you can see, all that extra irrigation is suctioned out and we've got our bone graft in the mesh bag. So after you're done harvesting your bone graft and you've irrigated your suction tubing, make sure and turn off your suction to decrease the vacuum on the container. And then simply counterclockwise turn of the lid allows the lid to be disengaged. This gives you access to the mesh bag in the center. Your bone graft still will be somewhat hydrated, so you can simply squeeze that mesh bag to get further liquid off of your bone graft. Once you're ready to get the bone graft out of the mesh bag, simply invert the mesh bag to expel your bone graft. You can put your fingers inside to tint up the mesh bag to allow you to scrape off any residual bone graft. There's a fair bit of bone graft here, and this is from just taking out a unilateral facet joint. Graphnet Excel not only allows you to collect a large amount of bone graft, but the handling characteristics allow for mobility. Obviously this then can be added to a substrate to increase your overall graft volume, such as PRP or DBM. So to illustrate how much bone graft you can get. Again, this is just from a unilateral facet harvest in a five cc syringe. We are able to decrease the overall cost and potentially less use of extenders.

GraftNet™ XL Bone Collection Device and Spine Fusion

Hi, I'm Andrew Clavenna. Thanks for tuning in. I'm here to talk to you about the role of autograft bone and spine procedures. If we look at the autograft bone, it is widely available. It's got our autologous cells and graft in a very cost effective. Typically, in our fusions, we're always trying to figure out what is going to yield us the best results, while also trying to look at the cost effectiveness of those graft materials we're using. The availability and quality of the bone graft has been looked at in multiple sources. In this study from 2018, it did show that the posterior spine bone dust, as it's called, does release these anabolic factors and potentially enhance the osteoblast proliferation. It is very easy to harvest this bone. It's simple, it's safe. And instead of throwing this bone away, we should be recycling this bone to allow our fusions to have better results. Looking specifically at that quality, there have been studies going all the way back to 2010 that do show that we have these mesenchymal stem cells that we need and allow our fusion to take is very robust. And therefore this is a great option when we're looking at fusions, looking at a systematic review collection of this autologous bone dust really has shown that as easily to collect the time of surgery and availability, to use it to augment our fusion, giving us the osteoinductive potential that we need to allow our fusions to take, and giving us those osteoprogenitor cells to proliferate and mineralize. There are studies looking at the Acdf model, and specifically, this study shows that using a mixture of bone dust with immortalized bone allowed for an earlier fusion and decreased cave subsidence. Obviously something that we all want for our patients. Looking at a case example from my practice, this is a 37 year old female medical editor that's been dealing with back pain essentially since her 20s. She's been managing this as much as she can through the years, and actually did a fairly good job for the past 15 years. Over the last two years, though, she started having increasing back end right more than left leg pain. She obviously went through all the conservative measures that we always do. Her pain is definitely worse with standing, walking, prolonged sitting and bending. Her x rays are typical for what we see with degenerative changes. Degenerative disc at the 4 or 5 segment disc space collapse. No significant deformity, no instability on our flexion extension x rays. And if you look at her progression over a six month period, you can definitely see that she has less motion through her spine due to this increased pain. Just simply in a six month period on flexion x rays and then progressive disc space collapse through that 4 or 5 segment. Her MRI is pretty classic. She has modic changes, bone edema on the sequencing. She has disk osteophyte into that low recess region and significant bilateral foraminal narrowing to enable us to allow her to return to increase function. This is a case where a decompression alone would not be possible to really obtain what she needs, and therefore she needs a fusion. Looking specifically at the bone dust that is able to be collected using the graphnet and the Fraser tip suction, the specimen cup shows the yield of that bone graft. These pictures illustrate our ability to modify that bone dust post collection. Whether you use a syringe or in the left picture, I really compacted the bone graft to give it more structure for easier insertion. Here are intraoperative X-rays showing that we have good disc height elevation through the use of mis-tlif. You can see the bone graft posterior to the cage. Here are six month post op pictures showing incorporation of that bone graft and no motion through that segment, and ultimately allowing her to return to function. Or on closer evaluation. Spot lateral does show that incorporation of bone graft to the vertebral bodies of L4 and L5. Graphnet XL bone collector allows for collection of autologous bone with a Frazier tip suction, allowing easy access to high volume. Bone graft. Has an inner mesh that allows for easy collection of this bone graft, and ultimately improves your sterile field workflow. This device is a great advancement for spine fusion procedures.
